A recent article published by the National Kidney Disease Clinical Research Center in the journal Amyloid, the journal of the International Amyloidosis Society, summarizes the efficacy of bortezomib combined with dexamethasone as a first-line regimen for the treatment of AL amyloidosis. The study included 72 patients, 46 men and 26 women, with a mean age of 56 years (37-76 years). 62 had λ light chain type and 10 had κ light chain type, all patients had renal involvement, 52 (72.2%) had cardiac involvement, 14 (19.4%) had hepatic involvement, and 7 (9.7%) had peripheral nerve involvement. 17 had renal involvement only, 39 had 2 In 17 cases, only the kidney was involved, in 39 cases, 2 organs were involved, and in 16 cases, 3 or more organs were involved. The average duration of disease before diagnosis was 11 months, and the main clinical manifestation was nephrotic syndrome. 17 patients had combined renal insufficiency. The mean duration of treatment was 2 courses (1-6 courses). 17 patients had drug dose adjustment or discontinuation due to drug side effects. 54 patients (75%) achieved hematologic response, of which 33 (45.8%) had complete remission (CR), 13 (18.1%) had very good partial remission (VGPR), and 8 had partial remission. The mean time to onset of hematologic response was 2 months. At a median follow-up of 22 months, patients had a 1-year survival rate of 82.9% and a 3-year survival rate of 74.7%, which did not reach the median survival. There were 34 renal reactions with a response rate of 47.2% and 21 cardiac reactions with a response rate of 40.3%. The main side effects of treatment were gastrointestinal reactions (48.6%), peripheral neurotoxicity (25%), infection (19.4%), thrombocytopenia (13.9%) and acute kidney injury (12.5%). Other rare complications include cardiac arrhythmias, intestinal obstruction, and heart failure. the BD regimen has a high hematologic response rate and organ response rate for AL amyloidosis, which improves patient prognosis, but complications during treatment need to be taken seriously.
