Final lying down in advanced stage of liver cancer may be due to the formation of a large amount of ascites, or it may be due to forced position caused by secondary heart failure or cancer pain. 1. Extensive abdominal metastasis may occur in advanced stage of liver cancer, which may lead to the formation of abdominal fluid. A large amount of ascites will cause the pressure in the abdominal cavity to rise greatly, squeezing the diaphragm and compressing the volume of thoracic cavity, leading to lung expansion insufficiency and dyspnea. When lying down, the pressure of ascites on diaphragm increases, so patients often need to take semi-recumbent position or sitting position in order to alleviate dyspnea to a certain extent. 2. Infection, electrolyte disorder, water and sodium retention in advanced stage of liver cancer may induce heart failure. When lying down, the return blood volume will increase, which will increase the burden of heart and cause dyspnea, so the patient needs to take a seated or semi-recumbent position to alleviate the symptoms of dyspnea. 3. Widespread metastasis of advanced liver cancer may cause persistent severe pain, which may be aggravated when lying down, and patients have to adopt certain forced position to relieve the pain.
