Gingival fistulas are not completely eradicated, mainly because local irritation is not eliminated and surgical excision is not complete, resulting in the persistence of infected lesions. If the underlying lesion is not removed, the gingival fistula can still return. For example, periapical fistulas are usually chronic and most patients have no painful reaction, so they are particularly easy to ignore, but the periapical area is actually inflamed all the time.
