Occasional cramps in the scrotal testicular area do not indicate the presence of organic pathology, and in most cases it is more likely to be a neuropathic pain caused by a nerve reflex. If the pain is caused by a disease of the testicular epididymis itself, it is often a persistent dull or vague pain, such as acute testicular epididymitis. Varicocele may also cause pain and discomfort around the scrotum, testicles and epididymis, which is also usually mainly persistent swelling and pain. If twitching pain occurs in the scrotal area, it can be further determined by scrotal ultrasound examination to clarify the possibility of intra-scrotal disease.
