Microwave ablation of uterine fibroids

  ☆ Indications
1 Symptomatic uterine fibroids with excessive menstrual bleeding and significant occupying compressive symptoms Jianxiong Wang, Department of Ultrasound, Nankai Hospital, Tianjin
2 Conservative treatment (including drug treatment and myomectomy) is ineffective or recurrence
3 Those who require preservation of the uterus and fertility
4 Tender submucosal fibroids of any size, single interstitial fibroids <5cm in diameter; multiple fibroids ≤3 in number and ≤3cm in diameter
  ☆ Contraindications
1 Malignant tumors of the uterus, such as uterine sarcoma, endometrial cancer
  2 Pregnancy
3 Acute and chronic pelvic inflammatory disease not controlled
☆ Informed consent for special treatment
The proposed ultrasound-guided percutaneous microwave ablation of uterine fibroids may present with.
  1, bleeding
  2. abdominal and pelvic infections, peritonitis
  3.Vaginal bleeding
  4.pain
  5.Postoperative fever
  6.Vacuum organ damage
  7.Anesthesia accident
8.The treatment (part) is not covered by medical insurance
  ☆ Technical points
1.The ablation range is less than 0.3cm from the edge of fibroids
2.For anterior wall fibroids, the probe should be slightly pressurized so that the fibroids are close to the anterior abdominal wall, which makes the puncture easy and safe.
3.For posterior wall fibroids, the puncture can be done through the anterior wall myometrium. The endometrium of the uterus in the path of puncture should be avoided as much as possible to avoid postoperative cavity adhesions or endometrial implantation.
  4. For subplasmic fibroids, attention should be paid to the complete ablation of the tip, especially the complete ablation of the trophoblastic vessels in the tip
  5. Ultrasonography should be performed after ablation to evaluate the ablation effect.
