Trigeminal neuralgia is an unimaginable pain that is known as the “number one pain in the world”. Therefore, it is easy for trigeminal neuralgia patients to seek medical help. At the same time, due to the lack of clinical treatment standard for trigeminal neuralgia, over-treatment is common, which makes some patients’ pain worse and even become intractable. In terms of the etiology of trigeminal neuralgia, 70%-80% is caused by vascular compression-type injury, while 20%-30% is caused by tumors, congenital malformations, inflammation of nerves and meninges, etc. Dr. Wang Yan said that regardless of the cause of trigeminal neuralgia, the treatment should follow two principles and three stages. One is the principle of going from non-invasive to minimally invasive and then invasive, and the second is the principle of going from conservative treatment to etiological treatment and finally palliative treatment. Electric stimulation for the treatment of trigeminal neuralgia From the principle of trigeminal neuralgia, Wang Yan classifies it into two types: nerve stem pain and central nerve pain. The former is more common and is primary trigeminal neuralgia; the latter is of medical origin and accounts for only 1%-2% of trigeminal neuralgia. For the central pain type of trigeminal neuralgia, there are only two treatments available, namely, massive doses of psychiatric drugs and spinal cord electrical stimulation therapy. Spinal cord electrical stimulation therapy (also called neuromodulation therapy) is an advanced treatment for chronic intractable pain by reducing or blocking the transmission of pain signals to the center through electrical stimulation of the posterior column of the spinal cord. The spinal cord electrical stimulation system consists of three components: electrodes implanted in the epidural space of the patient’s spinal cord, a stimulator implanted with subcutaneous electrical pulses in the abdomen or buttocks, and an extension lead connecting the two. After receiving the therapy, a small implanted system (similar to a pacemaker) emits a different sensation in place of pain. Some patients describe the sensation as being like a gentle massage, or making the pain less of a concern.
