Liu, female, 64 years old, retired, date of initial diagnosis: January 15, 2014. Chief complaint: dizziness and sleeplessness for 1 month. History: The patient had recurrent episodes of dizziness and nausea with difficulty sleeping in the past 1 month, and was seen in a Western medicine department. After taking hypoglycemic, antihypertensive and sedative drugs without significant relief, the patient was in pain and sought Chinese medicine treatment. The patient was suffering from dizziness and dullness, nausea and vomiting, sleeplessness, tinnitus and eye swelling, dry mouth and bitterness, cough with yellow sputum, shortness of breath and weakness, dullness and loose stools. Physical examination: blood pressure 140/90 mmHg, no other positive signs; dull tongue, greasy and slightly yellow coating, smooth and slightly counted pulse. Laboratory tests: FBG 7.6 mmol/L, LDL-C 4.6 mmol/L, TG 2.8 mmol/L. Past history: type 2 diabetes mellitus, hypertension, hyperlipidemia for 5 years. Western diagnosis: hypertension, hyperlipidemia, type 2 diabetes mellitus, insomnia; Chinese diagnosis: vertigo, evidence of spleen deficiency, liver wind, phlegm-heat internal disturbance. Treatment: strengthen the spleen, resolve phlegm, calm the liver and quench the wind, clear heat and calm the mind. Prescription: Chrysanthemum 60g, Fenfeng 20g, Hesperidin 3g, Gui Zhi 6g, Radix et Rhizoma Ginseng 20g, Atractylodes Macrocephala 20g, Poria 20g, Ginger 3g, Radix Angelicae Sinensis 6g, Chuanxiong 10g, Scutellaria Baicalensis 15g, Bai Qian 15g, Oyster 20g (first decoction), Mother of Pearl 40g, Licorice 3g 7 doses, 1 dose daily with water. Second Diagnosis (January 23, 2014): After taking the previous medicine, dizziness was relieved, sleep was easy, no nausea and vomiting, mental and physical strength improved, still dull, dry mouth and bitterness, stool was normal. The tongue is dull, with thin yellow coating and a small smooth pulse. The symptoms are greatly reduced, and the medicine is in the right place.
