Many people will experience gastrointestinal symptoms such as nausea, vomiting and bloating during pregnancy, and many pregnant women do not take this seriously, thinking that they will get better after a while. In fact, I do not know that these “gastrointestinal symptoms” may be the early manifestation of some serious diseases, once the delay in diagnosis, may lead to serious consequences, and even threaten the life of the mother and fetus. This is not an alarmist statement by the author! With the improvement of medical science, liver disease during pregnancy is now one of the most common and important factors leading to maternal death in China. As we all know, most pregnant women will heal themselves in the first month or so, and it rarely continues beyond the third month of pregnancy. If the vomiting is so severe that even drinking water is vomited and lasts for a long time, this kind of severe pregnancy vomiting can have adverse consequences for both mother and fetus. The most dangerous one is Wernicke’s encephalopathy induced by vitamin B1 deficiency, which can lead to the death of the mother and fetus; the second is the damage to liver function and kidney function. The second is the onset of various hepatitis during pregnancy, hepatitis B is the most common, as well as hepatitis C, hepatitis E, drug-related hepatitis, autoimmune liver disease, etc., which in the early stages mostly appear as gastrointestinal symptoms, with or without right upper abdominal distension and jaundice. If the early stage is not taken seriously and the treatment is delayed, the risk of maternal and fetal death increases greatly once the disease is delayed to severe hepatitis or liver failure. Again, acute fatty liver in pregnancy is a rare disease in late pregnancy. Most cases develop after the 30th week of pregnancy, and prior to 1990 the mortality rate was over 80%. In recent years, doctors have recognized this disease and it is generally accepted that as long as the disease is diagnosed in its early stages, the mortality rate is rapidly reduced with no sequelae, regardless of fetal survival, by immediate cesarean delivery. However, if the diagnosis is late, once the disease progresses to the stage of multiple organ failure, it is very difficult to resuscitate. In addition to nausea, poor nausea, vomiting, epigastric fullness, often accompanied by extreme weakness, insomnia, yellow urine, etc., this time must be immediately consulted, do not delay the diagnosis, are required to immediately cesarean section, 1 day can not be delayed, each day of delay, the threat of death will increase exponentially. Strangely enough, most of this disease can be fully recovered if the pregnancy is terminated in time. There is another type of liver disease during pregnancy called HEELP syndrome, which also occurs in late pregnancy and is often accompanied by hypertension, proteinuria, and edema. In this patient, in addition to nausea, vomiting, anorexia, and abdominal distention, it is often accompanied by pain and distension in the right upper abdomen and may be accompanied by jaundice and anemia. This is also a critical situation, and if the treatment is delayed, the life of the pregnant woman and her fetus may be in danger. In short, if these “gastrointestinal symptoms” occur in early pregnancy, they are not serious and can be overcome by regulating diet and other methods. However, gastrointestinal symptoms that occur in the middle of pregnancy, especially in the late pregnancy, as well as severe weakness and insomnia, must be highly alerted and are likely to be early manifestations of diseases that seriously threaten the life of the mother and child, and must be seen by a hospital immediately.
