[Abstract] Acromegaly is also known as familial osteochondrofibromatosis, familial acromegaly, maxillary enlargement, and familial multicystic jaw disease. It is also known as angel face disease because Jones (1933) first used the name cherubism (angel face-like disease). It is a rare disease that affects only the jaws of children, and is a separate condition because of its characteristic facial morphology and histological structure. [Some investigations have suggested that the disease is an autosomal dominant disorder. The exact pathogenesis is not known. The pathological essence of the disease is the replacement of normal bone tissue by abnormal fibrous connective tissue, which is rich in blood vessels, bone-like tissue or bone trabeculae, and rich in multinucleated giant cells. Usually, the vascular walls are thin and dilated, and the vessels are surrounded by eosinophilic red material in a cuff or cuff-like structure, and multinucleated giant cells often surround and adhere to the vessel walls. Direct fibrous osteogenesis or irregularly distributed new bone trabeculae are seen in the longitudinally and horizontally arranged fibers, the latter being heterochromatic and without normal deposition lines, and large foci of hemorrhage and iron-containing heme are often visible. In later stages, the fibrous component increases and osteogenesis is observed. [The lesion grows with age and body, but the development of the lesion may slow down or stop in adulthood. The lower jaw is usually more susceptible than the upper jaw, and because the lymph nodes are also enlarged, the patient has a swollen lower face, a full cheek and jaw, an elevated tongue, difficulty in speech, gaps in the milk teeth, missing and uneven teeth, and no teeth in heavy lesions. X-rays show that the jaws are swollen, the cortex is thin, and the interior shows a multi-cystic hypodensity area, which is limited to the molar area and ascending branch in mild cases, but the entire mandible or even the maxilla is involved in severe cases. [Treatment] Conservative surgical treatment is performed to improve facial shape and function after the lesion is quiescent.
