How much does emotion affect myasthenia gravis?

  Psychological factors as a cause of myasthenia gravis have been widely appreciated by the current academic community. Studies have shown that “trauma” is an independent risk factor for recurrence or exacerbation of myasthenia gravis. Although modern medical theory is difficult to find clear evidence and theory to explain the direct relationship between mental trauma and autoimmunity, it is easier to explain from the theory of Chinese medicine: mental trauma is equivalent to the category of emotional disease in Chinese medicine, which is specifically classified as “emotional discomfort”, “depression, anger and worry”. “When the emotions are not in order, it affects the qi flow of “liver”. If the liver is depressed, it is easy to violate the spleen and the earth; because the spleen is the master of the limbs and muscles, the master of transporting water and grain and transporting fluid and gas, the liver and wood will not be able to moisten the muscles of patients with myasthenia gravis, the gas does not follow the meridians and the movement is weak; therefore, the disease attacks or worsens.  Patients with myasthenia gravis have symptoms of central nervous system damage, which manifests as cognitive impairment. Since myasthenia gravis patients themselves have varying degrees of sleep disorders, sleep deprivation is one of the main causes of the onset of the disease. In addition, patients with myasthenia gravis are treated with stimulants, and one of the side effects of long-term use of stimulants is that they affect the central nervous system, which leads to cognitive fatigue; while long-term use of hormonal drugs leads to depression by disturbing the circadian rhythm of cortisol levels in patients.  For patients with myasthenia gravis who have developed the disease, they have to consider family, childbirth, and child support due to the fear of becoming a burden to their families and female patients. All these social reasons cause the occurrence of anxiety symptoms in patients with this disease, which in turn cause the disruption of endocrine level in patients’ bodies and make “mental exhaustion” an important factor in triggering the aggravation of this disease.  For patients who have undergone airway suctioning, tracheal intubation and intensive care in ICU, the fear of such invasive operations, resistance to mechanical assisted ventilation, speech communication disorder after intubation and loneliness of losing the company of relatives lead to their mental “allergy”, and they are still in a state of worry and tension for a long time after being discharged from the hospital. After being discharged from the hospital, they are still in a state of worry and stress for a long time.  In summary, most of the negative psychological state will make the human immune function easily disrupted, the body’s defense function is reduced, so that the myasthenia gravis induced or aggravated; at the same time, the attack and aggravation of the disease also makes the patient’s psychological state change, easy to tend to negative emotional development. Patients and their families should understand this situation as early as possible and maintain good communication with the patients in order to cooperate with the patient’s treatment plan and break the above vicious circle, so that the patients can obtain a good prognosis and a high standard of living as much as possible.
