Abstract: Mycoplasma infections are not uncommon clinically, especially pediatric mycoplasma pneumonia should be given adequate attention. Mycoplasma pneumoniae is neither a virus nor a bacterium, and the respiratory infections it causes must be treated with macrolide drugs to be effective. In this case, the child had a cough and fever that resolved but the cough remained unresolved. After being transferred to our hospital, the cough was significantly relieved by symptomatic treatment with cyclic erythromycin and other drugs.
Basic information】Female, 1 year old
Type of disease】Pediatric mycoplasma pneumonia
Hospital】Fudan University Hospital of Pediatrics
Date of Consultation】December 2021
Treatment plan】Medication (cyclic erythromycin tablets + amiloride oral solution + budesonide suspension for inhalation)
[Treatment period] Hospitalization for 1 week, followed by outpatient visit after 3 days
Treatment effect] Fever subsided, cough improved, and inflammation in the lung was significantly absorbed in the chest X-ray.
I. Initial consultation
The child had a cough with fever 2 weeks ago, and the cough and wheezing symptoms were obvious at that time, and the temperature was 39.7℃.
After admission, the child was given intravenous medication, daily azithromycin and ambroxol hydrochloride injection, with ibuprofen suspension to reduce fever and inhalation of budesonide suspension nebulizer inhalation, etc. The child’s fever returned to normal after 5 days of medication, but the cough symptoms were obvious, there was obvious sputum, and there was always wet rales on lung auscultation, the parents were uneasy and automatically discharged to our hospital. The diagnosis of pediatric mycoplasma pneumonia was clear and the treatment was effective.
II. Treatment history
At present, the child is recovering from pneumonia, and it is recommended to give cyclic erythromycin tablets in sequential therapy, followed by nebulization therapy to continue the outpatient follow-up. The diagnosis of mycoplasma pneumonia was clear, and the treatment with intravenous azithromycin and nebulized inhalation was effective. Considering the delayed clinical course of mycoplasma infection, we chose sequential therapy and suggested to continue oral erythromycin tablets with amiloride oral solution and inhalation budesonide suspension nebulized inhalation as a comprehensive treatment.
III. Treatment effect
Pediatric mycoplasma pneumonia due to mycoplasma infection is not uncommon in clinical practice. The child was treated satisfactorily with sequential therapy, including the administration of erythromycin tablets with amiloride oral solution and inhalation budesonide suspension by nebulized inhalation. After 1 week of hospitalization, the child was discharged from the hospital. 3 days later, outpatient follow-up showed that the child recovered well, the fever subsided, the cough improved, and the review of chest X-ray showed that the lung inflammation was significantly absorbed.
IV. Notes
The child is recovering well, and we are happy for the child and parents. Parents are advised to follow the doctor’s instructions and give their children regular medication during the outpatient follow-up period, try to avoid choking and coughing when eating, choose light and easily digestible food, and avoid greasy and cold food.
V. Personal insight
At present, mycoplasma infection is not uncommon in clinical practice, especially mycoplasma pneumonia in children should be taken seriously enough, we must prevent infection, go to public places less often, and improve our own resistance. If you have cough and fever and other uncomfortable symptoms, you must seek medical attention in a timely manner. In this case, after the clear diagnosis of mycoplasma pneumonia in the external hospital, the condition improved significantly after intravenous infusion of azithromycin, but because the absorption of lung inflammation needs a process, therefore, after clinical analysis and evaluation, the child was recommended to take sequential therapy for treatment and continue the clostridium erythromycin tablets, together with cough and sputum relief and other comprehensive treatment, and finally achieved satisfactory clinical results.
