After meticulous examination, combined with lower limb venous ultrasound and coagulation tests, the diagnosis of thrombotic superficial phlebitis was confirmed. After 10 days of anticoagulation and anti-inflammatory treatment, the patient’s redness, swelling and pain were significantly relieved and the skin temperature was normal. He was healthy in the past. The patient was found to have scattered red skin, high skin temperature, positive anterior tibial indentation, negative grip pain in the gastrocnemius muscle, palpable dorsalis pedis artery bilaterally, and fair sensorimotor function in both lower extremities. Ultrasonography was then performed, suggesting right femoral vein reflux, superficial phlebitis of the right calf, and thrombosis was not excluded, so the preliminary diagnosis was thrombotic superficial phlebitis, and the patient was recommended to be hospitalized. II. Treatment After admission, the patient was routinely given a blood biochemical examination, and no significant abnormalities were seen in the blood routine. Coagulation function suggested fibrinogen 3.61 g/L (normal value 1.8-3.5 g/L), so the diagnosis of thrombotic superficial phlebitis was confirmed. The disease was related to varicose veins for many years, but at present it was in the acute phase of inflammation, and rash surgery could easily cause non-healing of the incision. Therefore, after comprehensive assessment of the condition, the patient was advised to focus on anticoagulation, anti-inflammation, and decongestion in this hospitalization, and to wait for the acute inflammation to subside, and then undergo varicose vein surgery when the patient was reviewed in 3 months. The patient was then given low-molecular weight heparin sodium injection for anticoagulation, etimesine sulfate injection for anti-inflammation, dizocine injection for pain relief, poppy bases hydrochloride injection for vasodilatation, and oral mai zhi ling tablets to promote venous reflux. After the combined treatment of drugs, the patient’s symptoms were relieved after 3 days and significantly relieved after 1 week, and the pain disappeared after 10 days of medication, the skin temperature returned to normal, and the light pressure pain was pressed. However, patients should pay attention to the following matters when they are discharged from the hospital: 1, most of the thrombophlebitis is caused by varicose veins, and the disease has the characteristic of recurrence, so patients still need to pay attention to avoid sitting for a long time after discharge, and put their legs up when resting, and they must follow the doctor’s advice when taking the oral medicine to promote venous reflux, and review the disease on time; 2, after the acute inflammation of patients subsides, in order to avoid recurrence of the disease, if physical conditions If the physical condition allows, it is recommended to perform varicose vein surgery as soon as possible, so as not to delay and cause the aggravation of the disease. Personal insight The thrombotic superficial phlebitis of the lower limbs is mostly caused by varicose veins and venous insufficiency, which usually leads to redness, swelling, pain and other symptoms, so it is recommended that the acute phase of inflammation should be treated promptly with symptomatic treatment such as anti-inflammation, pain relief and promotion of venous reflux. Usually after symptomatic treatment, as in the patient in this article, the symptoms can be significantly improved. To avoid recurrence of the disease and even induce deep vein thrombosis, surgical treatment is recommended at an elective stage.
