Occult penis, also known as buried penis, is caused by less penile skin and abnormal attachment of the white membrane of the corpus cavernosum to the penile fascia. Clinical manifestations The appearance of the penis is short, the distance between the foreskin opening and the root of the penis is short, the foreskin looks like a bird’s beak wrapped around the penis and is not attached to the penis body, the dorsal side is short and the ventral side is long, the inner plate is more and the outer plate is less. Hold the penis with your hand while pushing the surrounding skin back, it can show the normal penile body. In obese children with fat accumulation in the lower abdomen, especially in front of the pubic bone, the penis can be occluded. The presence of supra-urethral cleft should be noted. In the case of suprapubic urethral cleft, a shallow groove can be palpated on the head of the penis. Treatment There is a great deal of controversy about the treatment and timing of surgery for occult penis. If the foreskin can be turned up to reveal the glans, surgery is not necessary and will improve with the development of the penis. For obese children, they can lose weight and control their weight first, but for typical children, surgery should be performed before the age of 5-7. 1, hormone therapy spongy body development is small, available HCG and androgen therapy. 2.Surgical treatment The aim is to enlarge the prepuce opening and expose the head of the penis. Care should be taken not to do simple circumcision. A surgical method ① Circumcise the outer plate along the circumcision opening, and cut the outer plate longitudinally along the 3 points of 2, 6 and 10 about 1.5 M. ② Cut the inner plate longitudinally along the 3 points of 4, 8 and 12 about 1.5 M, and the foreskin can be turned out to expose the head of the penis smoothly. The inner and outer foreskin plates are sutured together in an inset flap. B. Surgical method The foreskin is turned out as far as possible, and the narrow ring is incised longitudinally at 10 and 2 points, so that the head of the penis can be exposed smoothly by turning out the foreskin, and the inner plate is incised circumferentially about 0.8-1 M from the coronal groove, and the foreskin is trimmed and sutured.
