1.Oral anticoagulation therapy should be continued for 6 months or longer after discharge from hospital (depending on individual condition and 6-month review of angiography results). 2. Monitor the coagulation indexes continuously (preferably once a day or once every two days) for five days after the first oral Warfarin dose or change of dose, and observe the “INR (International Normalized Ratio)” value in the “PT (prothrombin time)” item. After five days, the monitoring interval should be changed to once every three days. When the INR value is stable between 2.0 and 2.5 in two consecutive checks and the difference is not significant, the monitoring interval can be extended to once a week, once every two weeks or once a month. Please also remember that the dose of Warfarin at this time is your safe and effective dose. 3.In the process of oral Warfarin, if the INR value is lower than 2.0 for two consecutive tests, you can increase 1/4 tablet to 1/2 tablet of Warfarin every day, and monitor the same way after changing the dose; if the INR value is found to be more than 3.0, you should stop taking the drug temporarily, and the INR value can return to normal after 4 to 5 days in general. 4.The first dose of oral Warfarin is based on your “safe and effective dose”, and the monitoring method is the same as the above two points. 5, after discharge from the hospital during the oral anticoagulation therapy must pay attention to observe whether the gums, oral cavity and nasal mucosa bleeding; observe whether there is a change in the color of urine (such as red indicates that there may be bleeding); observe whether there is a change in the color of stool, the color of stool is tarry when the gastrointestinal bleeding (black and shiny, smells bloody, if you are not sure, you can take an appropriate amount of stool and stir with water, if there is bleeding, the stirred water will turn red). If bleeding is found, stop taking Warfarin (or aspirin) immediately, and go to the hospital for corresponding examination and treatment. 6.Please bring your medical records and coagulation index test results to our department for follow-up consultation two weeks, one month, three months, six months and one year after discharge.
