Basic information about dilator treatment

Scar deformities of the face and neck caused by burns or other causes are common in plastic surgery. Not only does it cause cosmetic damage, scar contracture often leads to various functional impairments, such as restricted mouth opening, drooling, incomplete eyelid closure, and limited neck movement, which seriously affects the patient’s daily life. Many medical institutions in China still use skin grafts to treat facial and neck scarring. Although it can partially improve the symptoms, the color, texture and elasticity of the transplanted skin are not ideal. The skin expander method can effectively overcome the above-mentioned defects of skin grafting treatment, and the skin after repair has good elasticity, color, and texture matching with the surrounding area. For more than 10 years, our treatment team has been dedicated to researching the application of skin expander to treat facial and neck scarring. Through technical improvements and innovations, we have reduced complications such as infection and hematoma; expanded the treatment range of skin expander and improved the efficiency of expansion. For appropriate patients, facial scar including perioral and cheek scars can be repaired by skin expansion of the neck; for patients with no normal skin available for expansion of both face and neck, our group has developed a series of methods to repair facial and neck scars after distant skin flap expansion, such as: forehead, shoulder, lateral chest flap expansion, etc. For patients with large burns on the face, neck, and body that are difficult to repair with these methods, the group has developed a prefabricated flap expansion method for these patients. For some patients with severe facial organ destruction, it is even possible to create structures such as nose and lip in a distal position by prefabricated methods and then transfer them to repair the face.
