Ovarian teratoma presents on CT as a multiple cystic lesion with a capsule within a capsule in most cases. The signal or density of each capsule is different, as are the contents of each capsule. The common imaging findings are mainly fat and fluid contents, with some characteristic density signal. In this case it is suggestive of teratoma, and some of the sacs may be seen as nodules or masses with cephalic formation. If the nodules in the wall of the capsule are relatively large and mass-shaped, with irregular margins and extracapsular invasion there is a possibility that they are malignant. Enhancement scans may show mild or moderate enhancement between the cyst wall and the nodule, and a comprehensive judgment and subsequent management will be given based on the corresponding CT performance, as well as the corresponding tumor markers and clinical manifestations in women.
