Abstract: This patient presented with abdominal discomfort, normal menstruation and no dysmenorrhea or abdominal pain. On examination, the patient was found to have a 9-cm-sized mass in the pelvis with a relatively smooth surface and clear borders, and the mass was oval with strong echogenicity and thick cystic wall under ultrasound, which was diagnosed as teratoma. After laparoscopic resection of bilateral ovarian teratomas, the patient had complete detachment of teratoma and recovered well after surgery.

I. Initial consultation
The patient visited the clinic because of abdominal discomfort. The examination revealed an 8 cm-sized mass in the pelvis, and the ultrasound examination showed that the right fallopian tube was not clearly visible, and a 9 cm-sized mass could be seen in the right adnexal area. The patient complained of normal menstruation, no dysmenorrhea, abdominal pain and other symptoms, no discomfort in general, and no history of gastrointestinal disease or surgery.
II. Treatment process
On examination after admission, it was seen that the surface of the mass in the pelvic cavity was relatively smooth and the boundary was clear. The mass was oval with strong echogenicity and thick cystic wall under ultrasound, which was considered as teratoma, but the mass progressed slowly, and the surface was relatively smooth and the boundary was clear, so it was more likely to be benign. After combining the patient’s medical history and relevant examinations, bilateral ovarian teratoma resection was performed laparoscopically on the second day of admission. The operation was very smooth and the cysts did not rupture during peeling.
III. Treatment effect
After minimally invasive surgery, the patient’s pelvic mass was completely removed, and the operation went very well. The pathological results showed that the tumor was benign, and the patient was able to move freely after the operation.
IV. Precautions
After surgical treatment, the patient’s teratoma was completely peeled off and no other organs showed any obvious abnormality, and a satisfactory treatment result was obtained. Patients should pay attention to rest after discharge, avoid strenuous exercise and heavy physical labor in the early morning after surgery, and pay attention to nutrition, eat more high-protein food, fresh vegetables and fruits, and forbid spicy and stimulating food, and after recovery, exercise properly to strengthen their immunity, and conduct regular review.
V. Personal insight
Benign tumor usually grows slowly and has no uncomfortable symptoms at the early stage, so it is not easy to be found, usually it is found during physical examination. In this case, the patient did not detect benign tumor because of long-term physical examination, and only after the appearance of discomfort symptoms did she seek medical treatment, but fortunately, after surgical treatment, the teratoma was completely peeled off and the patient’s abdominal discomfort disappeared, and she obtained satisfactory treatment results.
