Many people neglect post-operative protection causing re-trauma to the vocal cords, making voice problems more serious and increasing distress. The following is a summary of postoperative precautions for voice diseases in our department now, which we hope can help you. 1. Postoperative antibiotic application. Application method: Both oral and intravenous injection are available. Application time: Considering the possible abrasion of the pharynx and tongue root, the recommended application time is 3-7 days. Type of application: conventional broad-spectrum antibiotics such as penicillins, cephalosporins and macrolides are available. If the surgery goes smoothly and the surgical trauma is minor without causing mechanical damage to the pharyngeal cavity, antibiotics may not be applied. 2.Postoperative nebulized inhalation. Application time: 7-14 days, 2 times a day. Application of drugs: Pramipexole or Gentamax + Demi. Postoperative nebulized inhalation is preferred to pramipexole, a small dose of short-acting hormone specifically for nebulization, which can reduce inflammation of respiratory mucosa and reduce mucosal edema of the vocal cords with minimal side effects. The effect should be better than Qingda + Demi, but considering that patients in some areas cannot buy it, some patients still apply Qingda + Demi. Then there is the choice of nebulizer. My advice to voice disease patients is to buy one, not that we will rely on it in the future, but it is an essential piece of equipment. To take a step back, voice patients are still abusing their voices after surgery, and it is good for their voices if they do several nebulizations in time to reduce vocal cord edema. Now the nebulizer machine is divided into two kinds of compression nebulization and ultrasonic nebulization. Medical equipment stores and a treasure are available, do not buy too cheap, about 200-300 yuan should be able to meet the requirements. The compressed nebulizer may be better, and the nebulizing equipment for hospitalization is common, so it is recommended to buy. 3, postoperative sound ban. This point is a bit confusing, many local hospitals recommend 2-14 days after surgery to start pronation, while we recommend 30 days. Why 30 days? Many patients reflect that the time is too long and they can’t bear it, but our advice to them is that they must bear it. Most of the known voice diseases are caused by voice abuse, so letting patients speak 30 days after surgery is a punishment for their preoperative abuse, so that they can experience what it is like not to be able to speak, and then they will naturally control their pronunciation. However, speaking 30 days after surgery still brings confusion to us. Some patients respond that they can’t speak anymore, which requires voice training. We usually ask patients to return to the hospital for review after 1 month, and correct their pronunciation problems while reviewing, so that they can recover their original voice. 4. Postoperative anti-reflux drugs. It is now proven in many literatures that voice disorders and reflux are significantly related, so postoperative anti-reflux is essential to promote the recovery of vocal cords. Application of drugs: proton pump inhibitors and 5-HT4 agonists . To put it simply: drugs to inhibit reflux and drugs to promote gastrointestinal activity. After our repeated trials, the specific drugs are: omeprazole and mosapride, also according to the condition of different patients apply different drugs. Application time: If there are obvious reflux symptoms before surgery, it is recommended that the minimum application time after surgery is 3 months. If there is no preoperative reflux symptom, postoperative application for 1 month, and then consider whether to continue the application after the first review. Specific dose of omeprazole 20-40mg orally before breakfast and dinner. Mosapride: 5mg orally Three meals before.
