Ultra-minimally invasive funnel chest orthopedic surgery

Excavatum is the most common congenital thoracic deformity, manifesting as a depression of the anterior chest wall centered on the lower and middle sternum, shaped like a funnel, and worsening with growth and development, the depressed sternum compresses the heart and lungs, affecting the development of organs, producing symptoms such as respiratory tract infections and reduced activity endurance, and causing great psychological stress to the child and parents. Surgical correction is the only option to improve the condition. In the past, sternal reversal, rib cartilage excision or severance were used, which required extensive separation of the subcutaneous and muscles and severance of the sternum and ribs. The operation takes a long time, is traumatic, bleeds a lot, has obvious scars, and causes postoperative thoracic developmental disorders and chest wall sclerosis. Over the years, we have treated hundreds of cases of congenital funnel chest and recurrent funnel chest using thoracoscopic-assisted minimally invasive funnel chest correction (NUSS surgery), with a success rate close to 100% and very satisfactory results, ranking first in China. In the past two years, based on the success of NUSS surgery, domestic scholars have been exploring medical devices that can simplify the surgery and reduce the damage, and finally some clinical experts, based on the advantages and disadvantages of clinical NUSS surgery, have developed an ultra-minimally invasive funnel chest orthopedic steel plate with independent intellectual property rights, the application of which makes the funnel chest orthopedic surgery simpler and less traumatic for the patient: further shortening The application of this plate makes the funnel chest orthopedic surgery simpler and less traumatic for the patient: further shortening the operation time, improving the surgical effect and success rate, reducing the surgical injury and postoperative complications, reducing the postoperative pain, reducing the operation cost, shortening the hospital stay, and expanding the surgical indications. Compared with traditional NUSS surgery, ultra-minimally invasive funnel chest correction has the following major advantages: (1) there is generally no need to make muscle and subcutaneous passages in advance, and through clever design, the plate and guide can be passed in one pass, avoiding the damage to the tissue from repeated passage of the guide, and there is almost no gap between the plate and the tissue, so that the plate is not easily displaced to achieve better orthopedic results; (2) the pioneering design of the plate (2) the pioneering design of the steel plate makes it unnecessary to flip the plate as the existing traditional plate, thus avoiding the huge lacerations to the soft tissues of the chest wall caused by the flip; (3) the use of the snapping technique makes the installation of the plate fixation piece more convenient, fast, safe and precise. (4) The plate is placed on the anterior chest wall, which effectively solves the limitation of the traditional plate on the lateral development of the patient’s chest, so that the plate can be placed in the body for a longer time to achieve better orthopedic effect; the plate is supported by the rib bone and intercostal muscle together, and the support force is sufficient so that the plate will not collapse, thus achieving a satisfactory orthopedic effect. The patient’s postoperative pain is substantially reduced and lasts for a shorter period of time, and the postoperative sleeping position is not as demanding as traditional NUSS surgery, allowing turning and lying on the side; (5) the incision is in the anterior axillary line, making the muscle and subcutaneous tunnels shortened by more than half, making the separation of thoracic adhesions easier in patients with postoperative recurrence and causing less damage to mediastinal tissue; (6) truly providing individualized treatment for asymmetric (7) the operation time is halved, no chest tube is placed, and the patient can be discharged in 2-3 days after the operation, and the cost is saved by about 20% compared with the previous one. The development of ultra-minimally invasive funnel chest correction marks the international leading level in the treatment of funnel chest in China.
