What is the rehabilitation exercise after polydactyly surgery?

  After polydactyly surgery, it is important to pay attention to functional exercises. In complex polydactyly, it is also important to insist on wearing a palm rest and massage to assist in the correction of the deformity. But which children need functional exercises and palmar braces? What do I need to pay attention to when exercising and wearing a palmar brace?  Functional exercises are needed after surgery because the tendons are moved during surgery. Except for soft tissue polydactyly, which only requires removal of the polydactyly, for other types of polydactyly, tendon displacement and reconstruction of the abduction function are also done during surgery. In order to have better function of the preserved finger, functional exercises of the thumb should be done after the stitches are removed two weeks after surgery. The main functions of the thumb are extension, flexion, abduction and adduction (as shown in the figure). Postoperative exercises are needed mainly for the extension and flexion of the thumb, because the functions of abduction and adduction have nothing to do with the heel joint and mainly rely on muscle strength to achieve them, and generally do not need to be practiced. The function of extension and flexion is achieved by the tendon and the joint. After surgery to displace the tendon, postoperative exercises for extension and flexion should be done to enhance the function of finger movement to avoid stiffness of the joint.  Insist on wearing palm rests and massage to possibly avoid secondary surgery In some children with complex polydactyly, if there are residual deformities in the joints of the root and interphalangeal joints after surgery, they can be corrected by wearing palm rests and massage. By wearing a palmar brace and keeping the thumb extended like an extensor, the joint deviation at the base of the finger can be corrected. The same is true for interphalangeal joint deviations. By wearing a custom-made palm brace that limits the direction of growth of the deviated finger, the finger will grow straighter and straighter over time. The palm brace is generally required to be worn for 24 hours as much as possible, during which time it can be properly removed and cleaned, and also massaged on the deformed area. There are no special requirements for this kind of massage, it is mainly to press and knead the skewed parts. It is like correcting a small tree that has grown crooked, by using a wooden stick to top it and make it grow in a good direction. This is also true for residual deformities, artificially affecting the growth trend of the child’s bones, and most children are able to avoid secondary orthopedic surgery. The palm brace is usually worn consistently and massaged for six months, and can be removed when you see significant improvement in the appearance of the finger.  It is a long process for children to wear a palmar brace, and parents need to be careful to observe the process. The palm rest has many small eyes, breathable, so directly on the hand on it, no need to add other things inside, such as gloves. However, the palm rest is after all made of hard functional plastic, and skin contact friction for a long time, it is easy to cause skin pressure and wear, parents need to be careful care.
