“Treating the disease before it occurs” is an important prevention and treatment idea in Chinese medicine, which mainly includes three aspects: nurturing the disease before it occurs and preventing it before it occurs, treating the disease early and preventing its transmission, and regulating the disease after it occurs and preventing its recurrence. Prof. He Jialin is the third and fourth batch of academic experience instructors of old Chinese medicine practitioners in China, and has been devoted to gynecology for more than 40 years. The author has the honor to join the introduction and summarize the experience of Professor He in using the theory of “treating the untreated” to guide the treatment of gynecological diseases, which is introduced as follows. 1, before the disease to maintain health, prevention of disease in the first “treatment before the disease” should first include active measures to prevent the occurrence of disease before it occurs. Su Wen? Shanggu Tianzhen Lun”: “Tranquility and emptiness, the true energy from it, the spirit within the guard, the disease is safe from.” “The one who knows is the one who is in harmony with the yin and yang, the one who is in harmony with the arts, the one who eats and drinks in moderation, the one who lives and works in a normal manner, and the one who does not work in a delusional manner, so that he can be in shape and in spirit, and end his days.” He Shi clinical advocate “prevention is more important than cure”, pay attention to disease health publicity and education, pay attention to the diet, living, mood and other aspects of targeted guidance. 1.1 Dietary regimen He Shi’s clinical evidence focuses on physical identification “treating the disease before it happens”. Excessive dieting and weight loss can lead to late menstruation and even amenorrhea, and amenorrhea and infertility are also common in those who are obese and have a taste for fat and sweet. For patients with thin and weak menstrual disorders, Dr. He instructs them to resume a normal diet, and at the same time treats them with medicines to strengthen the spleen and kidneys; while for patients with obese and phlegm-damp constitution, he instructs them to eat a moderate diet, control their weight, and treat them with medicines to benefit the qi, resolve phlegm and eliminate fat. Dr. He emphasized that pregnant women should focus on dietary supplementation therapy. “Postpartum Qi deficiency and blood deficiency, the veins and ligaments are empty, the limbs are slack, the couples are open, the skin and hair are not solid, the camp guard is not solid, and the blood channels are easily blocked, so it is easier to cause illnesses and more difficult to remove them than this.” He instructed pregnant women to eat nutritious and easily digestible food based on the principle of “cool before birth and warm after birth”. He often recommends food therapy such as steamed light vegetables with fire heel to prevent postpartum plantar pain and lumbago. 1.2 Regularity of living and resting He has repeatedly warned women to pay attention to hygiene before and after menstruation. Sexual intercourse, swimming and tub bathing are prohibited during menstruation, one or two days before and after menstruation to prevent upstream infection leading to endometritis, pelvic inflammatory disease, tubal obstruction and other diseases. He also pointed out that women should pay particular attention to the regulation of life and living during menstruation. Primary dysmenorrhea is a common gynecological condition. Patients usually have no conscious symptoms as normal people, but usually the abdominal pain is more intense on the first day of menstruation, often accompanied by nausea and vomiting, even vomiting and diarrhea, fear of cold and curling up, and cold extremities. The clinical evidence is that cold clotting and blood stasis is the most common type, and the cause is often excessive consumption of cold or cold in the water during menstruation. Therefore, the menstrual period should control the cold diet, avoid taking cold baths and getting involved in the rain. The treatment is to warm the menstruation and disperse cold, invigorate blood and regulate flushing. The prescription is: Radix et Rhizoma Pseudostellariae, Cinnamon, Cornu Cervi Pantotrichum, Aconite, Radix Angelicae Sinensis, Rhizoma Chuanxiong, Rhizoma Safflower, Rhizoma Peach, Rhizoma Azadirachtae, Green Peel, Ocimum sanctum, Fried Yuan Hu and Roasted Licorice. It is also emphasized that the medicine should be taken in time before menstruation, i.e. 3~4 days before the expected next menstrual flow, which is more effective. Usually, you can take menstruation pills, etc., and then take the front 4 days before the next period. Generally, if taken for 1~3 months, dysmenorrhea can be improved significantly. 1.3 Regulation of emotion and will He believes that the factor of emotion and will plays an important role in the etiology and pathogenesis of gynecology in Chinese medicine. Su Wen? It is said in “The Theory of Raising Pain” that “all diseases are born in qi, anger is qi up, happiness is qi slow, sadness is qi down, fear is qi down, fright is qi chaos, thought is qi knot.” Suwen? yin and yang should be like the great theory of cloud: “anger hurts the liver”, “happy sad”, “thinking hurts the spleen”, “worry hurts the lungs”, “fear hurts the kidneys”. “Fear hurts the kidneys”. Adverse emotional stimuli can cause disease alone or lead to the deterioration of the condition. A good state of mind and stable emotions are the first foundation of gynecological “treatment of the untreated”. Su Wen? Yin and Yang, “the two yang disease of the heart and spleen, there is no hidden curve, women do not moon.” The clinical treatment of Dr. He’s patients is to provide the necessary psychological counseling for the causes of menstrual disorders, amenorrhea and gonorrhea, and to divert their attention, combine work and rest, and try to relax. She also added the following herbs to nourish the heart and liver and strengthen the spleen: Huai Mai, Roasted Licorice, Yuan Zhi, Wu Wei Zi, Radix Angelicae Sinensis, Astragalus, Poria, White Peony, Radix Aromaticus, and Yu Jin. Psychological infertility is a special type of infertility. They are often eager to get pregnant, but the desire for speed is not enough. Interestingly, it is common clinically that they actually become pregnant on their own after they are ready to give up and adopt a child. On the one hand, the psychological infertility tendency of the He Division to advise their diet and living in a normal manner, relax, let nature take its course, must not be too anxious, on the one hand, the discretion to use calamus, tulip, green plum blossom, nightshade vine, hops skin, sour dates, Yuan Zhi, Wu Wei Zi, Huai wheat, roasted licorice and other open-hearted and tranquilizing products. 2. Early treatment of the disease and prevention of its transmission 2.1 Early diagnosis and treatment Although the disease has not yet occurred, but there have been some aura, or in the budding state, measures should be taken to prevent the disease from occurring. In the early stage of the disease, the disease is generally shallow, the condition is light, and the damage to the positive energy is not very serious, so early treatment can achieve the purpose of easy treatment. In the Jin Kui Essentials? The first of the pulse evidence of successive diseases of the internal organs and meridians: “The appropriate meridians, before the flow of the internal organs, that is, the medical treatment. Extremities before feeling heavy stagnation, that is, guidance, vomiting, acupuncture, moxibustion, paste, do not make the nine orifices closed.” This is to emphasize the early treatment of disease. In the clinical practice of TCM gynecology, Master He has a lot of experience in this kind of application and is well versed in it, with the following examples: (1) After conservative drug treatment for ectopic pregnancy or after conservative surgery. There is a high possibility of ectopic pregnancy occurring in these patients when they try to conceive again, and about 1/3 can have another ectopic pregnancy and another about 1/3 later become secondary to infertility [1], and the patient’s HSG angiography often indicates that the fallopian tubes are open but not smooth. He’s Tongduan Tang was administered orally with the prescription consisting of Deer horn tablets, Andrographis paniculata, Astragalus membranaceus, Angelica sinensis, Chuanxiong rhizome, Red vine, Sulforaphane, Sanguine, Curcuma longa, Soapberry and Lulutong. Combine with extra-feminine IV enema (composition: red vine, septoria, white flowered snake’s tongue, angelica, red peony, frankincense, neem, etc.) to retain the enema. If necessary, it is supplemented by hysteroscopic tubal intubation and lavage. This combination of internal and external treatment and three tubes (oral medication in the esophagus, reserved enema in the anal canal and tubal intubation and lavage) greatly improves the success rate of pregnancy and reduces the risk of recurrence of ectopic pregnancy. (2) Pre-pregnancy conditioning and post-pregnancy antifertility treatment for recurrent spontaneous abortion. Repeated spontaneous miscarriage refers to those who have had 2 or more consecutive spontaneous miscarriages, and in Chinese medicine, miscarriage for more than 3 times is called slippage. This disease is an advantageous disease in He’s gynecology and has accumulated rich clinical experience. He believes that most of the cases are due to maternal congenital deficiency or acquired damage; or due to loss of qi and blood, unable to shade the fetus; or short interval between conceptions and loss of nourishment of the fetal element, resulting in repeated pregnancies and abortions. The treatment should focus on finding the cause, treating both men and women together, and providing pre-conception care to nourish the congenital body and regulate the impetus, so as to avoid the risk of repeated abortions during pregnancy. The kidney is the origin of the innate nature, harboring sperm and mastering reproduction, while the spleen is the origin of the postnatal nature, the source of biochemistry of qi and blood. Insufficient spleen and kidney, the essence and blood deficiency, the lack of nourishment and difficult to bear the fetus. Therefore, to regulate menstrual seeds, the first priority is to tonify the kidneys and strengthen the spleen. The prescription is based on the Yulin formula (experienced formula) with the addition and subtraction of Radix et Rhizoma Ginseng, Radix Astragali, Rhizoma Atractylodis Macrocephalae, Rhizoma Yam, Radix Angelicae Sinensis, Rhizoma Chuanxiong, Radix Rehmanniae Praeparata, Fructus Lycii, Rhizoma Cistanches, Semen Cuscutae, Rhizoma Raspberry, Semen Spleen, Radix Bacopa Monnieri, Radix Eucommiae Praeparatae, Radix et Rhizoma Conyzae. If the shape is fat and phlegm-damp, add Jiang Hanxia, Chen Chancellor Xing, and Huanghong; if the liver is depressed, add Chai Hu, Yu Jin, Xiang Shen, and Qing Pi; if the uterus is cold, add Cinnamon, Radix et Rhizoma, Ai Ye, and Cumin. For yin deficiency and blood heat, add Radix et Rhizoma, Cornu Cervi Pantotrichum, Macrocephalus, Agaricus, Scutellaria and Rhizoma Polygonatum. After pregnancy, strengthening the spleen and tonifying the kidneys to calm the fetus is also the main method. Prescription: Radix Codonopsis pilosulae, Radix prunus ginseng, Rhizoma Atractylodis Macrocephalae, Radix Scutellariae Sinensis, Radix Angelicae Sinensis, Radix Paeonia lactiflora, Radix et Rhizoma Chuanxian, Radix et Rhizoma Eucommiae, Semen Cuscutae, Radix Aconite, Rhizoma Ramie. For those with vaginal leakage of redness, add Radix Rehmanniae Charantia, Radix Lotus Root Charantia, Semen Cyperus, Dry Lotus Grass; for those with obvious nausea, add Radix Sulforaphus, Radix et Rhizoma Sandwelliae, Pericarpium Citri Reticulatae; for those with obvious kidney deficiency and low hormone level, add Radix Bacopa Monnieri, Zizyphus. (3) ABO blood group incompatibility. Neonatal hemolytic disease caused by maternal and child blood group incompatibility is an immune disease, which can lead to fetal or neonatal death in severe cases. Among them, ABO blood group incompatibility is the most common, accounting for 96% of neonatal hemolytic diseases in China [1], while Rh blood group incompatibility is less common clinically. The importance of testing for ABO blood group antibodies after pregnancy is now widely recognized. Modern medicine uses immunosuppression, blue light irradiation, and blood exchange for the treatment of established neonatal hemolytic disease, but they are all remedial measures with poor results and high economic costs. For mother and child with ABO blood group incompatibility, clinically, there are often only high serum antibody potency and no obvious conscious symptoms, so Master He combined disease identification with evidence identification and applied the method of clearing dampness and calming the fetus, i.e., Yin Chen Artemisia Tang with flavor. Prescription: Radix et Rhizoma, Rhizoma Rheum, Rhizoma Gardenia, Radix Scutellariae, Rhizoma Ramie, Radix Astragali, Atractylodes Macrocephalae, Rhizoma Atractylodis Macrocephalae, Radix Magnoliae Sinensis, Radix Eucommiae Praeparata, Radix Angelicae Sinensis, Radix Su Stem, Pericarpium Citriodorus. After the middle and late pregnancy, those with high serum antibody potency can add blood cooling and stasis-clearing drugs such as Danpi, Salvia, and Red Peony to improve placental blood viscosity and nourish the fetus. Through the follow-up observation of hundreds of clinical cases, the effect is satisfactory. 2.2 Firstly, when treating diseases, Master He also pays attention to accurately predict the tendency of disease transmission according to the law of disease development and transmission, and take preventive measures to stop its development and transmission. In the Book of Difficulties? The Seventy-seven Difficulties” says: “The so-called cure for the undiseased is to know when the liver disease is transmitted to the spleen, so first strengthen its spleen qi, so that no evil from the liver will be received, so it is said to cure the undiseased”. He is unique in the application of Chinese medicine in the treatment of postpartum malignant dew and vaginal bleeding after medical abortion. After delivery and miscarriage, the rudiments are damaged and susceptible to evil toxins, causing stasis in the uterus and blood not returning to the menstruum, resulting in vaginal bleeding. The clinical manifestation of the patient is that the malignant dew does not stop, the amount is more or less, the color is bright, the texture is thick and lumpy, and sometimes it can be accompanied by vague pain in the abdomen, which is equivalent to modern medicine’s endometritis, or even adnexitis and pelvic inflammatory disease. Immediately after childbirth, the treatment is with addition of biochemistry soup. Commonly used drugs include angelica, chuanxiong, motherwort, peach kernel, gunpowder ginger, rhubarb, danpi, horsetail, lotus room, blood surplus charcoal, red vine, septoria, fleabane, and loss of laughter to prevent pelvic inflammatory disease. Master He’s experience is that it is advisable to use heat-clearing and stasis-removing drugs early, which is equivalent to the prophylactic anti-infection in modern medicine and can reduce the long-term complications after delivery and abortion. 3. Post-millennial regimen to prevent recurrence “treating the disease” should also include post-millennial regimen, taking various measures to prevent recurrence of the disease. Although the symptoms disappear during the post-milbital stage when the disease is first healed, the evil energy is not yet exhausted, the positive energy is not yet restored, and the function of the internal organs is not yet robust, so it must be regulated before it can gradually recover. After recovering from the disease, we can consolidate the efficacy of the treatment with medicine, and at the same time with dietary regimen, in order to recover early and avoid the recurrence of the disease. Master He also has rich experience in the post-mortem regulation of gynecological diseases. 3.1 Tonifying the kidney and regulating the circumference, clarifying the source and restoring the old is the basic method of treating the leakage. “At the beginning, the treatment is to stop the bleeding to stop the flow; in the middle, it is to clear the heat and cool the blood to clarify its source; at the end, it is to tonify the blood to restore the old. If only to plug its flow, not to clarify its source, then the monstrous potential can not be curbed; if only to clarify its source, but not to restore its old, then the lonely Yang can not be established, so the end is not left behind, before and after the disorder, only to say that the treatment.” Therefore, after the bleeding has stopped, we should focus on clarifying the source and restoring the old, restoring normal menstruation and preventing the recurrence of bleeding. “Menstrual water comes out of the kidneys”. The root of the disease is kidney qi, the change is in tiankui, the location of the disease is in the punch and the blood, and the testimony is in qi and blood, which is manifested in the blood coming down from the uterus from time to time, either as collapse or leakage, or both collapse and leakage. Therefore, Chengyuan restoration should be based on tonifying the kidneys, while treating Qi and Blood together and taking into account the five organs, in order to adjust the menstrual cycle and restore normal ovulation function. The drugs commonly used by Dr. He in the clinical treatment of Chengyuan rejuvenation of menorrhagia are: deer horn cream, tortoise plate, Xianling spleen, Semen cuscutae, raspberry, jinzhuzi, Sha Yuanzi, Fructus Lycii, mulberry, fried eucommia, prunus ginseng, astragalus, raw groundnut charcoal, ripe groundnut charcoal, angelica, white peony and so on. At the same time, combined with herbal cycle therapy, after menstrual purification, we favor nourishing the blood, nourishing the kidney and filling the essence, during the intermenstrual period, tonifying the kidney to promote ovulation, and during the premenstrual period, warming the kidney and Yang to maintain the luteal function. 3.2 Nourishing the kidney to break the stasis and prevent recurrence after surgery Endometriosis (endometriosis) is a common and difficult disease in gynecology in which the endometrial tissue with growth function is planted outside the uterine cavity. In recent years, a lot of research work has been carried out on its etiology, diagnosis, treatment and prevention, and many achievements have been made, but its pathogenesis is still not very clear, the treatment results are still unsatisfactory, and the recurrence rate is high, with a recurrence rate of 20%-40% at 5 years after surgery [1]. Since the early 1990s, Dr. He has started a special outpatient clinic for endometriosis, and under the guidance of the theory of “treating the untreated disease”, he has been treating endometriosis with Chinese herbal medicine to help correct and resolve stasis after surgery, and has achieved better results. In addition to treating this disease by activating blood circulation and removing stasis to relieve pain, he also added drugs to support the kidney. To break the stasis and disperse the nodules should follow the principle of “the big accumulation and the big gathering should be stopped by decaying most of it”, avoiding violent attacks and harsh attacks to avoid damaging the positive energy. The medicine should be divided into menstrual period and usual period, when menstruation should be prevented from large amount of menstrual pain, to warm the menstruation, eliminate stasis and relieve pain, with Neiyi No. 1 formula, the medicine selected from Astragalus, Angelica, Chuanxiong, Red and White Peony, Sanliao, Curcuma, Lost smile, Jiaozha char, Yuanhu, Xiejian char, squid bone, frankincense, myrrh, etc.; usual period should be tonified kidney to break stasis and eliminate Y, with Neiyi No. 2 formula, the medicine selected from Deer Horns, Panax notoginseng, Xian Ling Spleen, Cuscuta, Red Vine, Fructus, Sanliao, Curcuma, cat’s claw, etc. Curcuma longa, Cat’s claw herb, Semen lily of the valley, Angelica sinensis, Chuanxiong rhizome, rice rhizome, Poria, Zeligia, etc. In cases of cold and damp stagnation, warming the menstruation, dispersing cold and relieving stasis, adding fenugreek, cumin, etc.; in cases of combined pelvic inflammation, adding Danpi, Danshen, rhubarb, etc.; in cases of mixed internal heterogeneity, adding cat’s ginseng, Xia Gu Cao, soapberry, seaweed, etc.
