Should pigmented moles be treated or not? Pigmented moles are everywhere. Most of us in the yellow race find pigmented moles on the face or other parts of the body at one time or another. Pigmented nevi are extremely common in plastic and cosmetic surgery. Some people do not come to plastic surgery because they do not know about laser or surgical treatments in plastic surgery, or they have a fear of hospitals, or they have less knowledge about the subject, or they do not come to plastic surgery for facial considerations. The question on everyone’s mind is whether pigmented moles need to be treated or not? What kind of nevus needs to be treated? What treatment methods are available? What kind of moles are suitable for laser treatment and what kind of moles need to be surgically removed? Now I will take you to the initial understanding of the knowledge of pigmented nevus: In common parlance, it is an abnormality of the metabolism of melanocytes. The general diagnosis of pigmented nevus is easy, but for some special types need to be carefully diagnosed and identified. The diagnosis of nevus is usually easy, but for some special types, the diagnosis and identification should be made carefully, or early surgical pathology examination should be clearly defined, because a few nevi are at risk of malignant changes, and need to be distinguished from melanocytoma. Some huge nevi or pigmented spots are not included in the scope of this section. According to the level of distribution, nevi can be divided into intradermal nevus, junctional nevus and mixed nevus. Intradermal nevus is deeper, commonly seen in adults, hemispherical elevated, brown or skin-colored small swelling, smooth surface, sometimes with one or several hairs in the center. Most of them are less than 1 centimeter in diameter. Junctional nevus is located at a slightly shallower level, appearing in infancy or childhood, and may be manifested as a spot or slightly elevated, smooth and hairless, with a diameter of no more than 0.8 centimeters. Mixed nevus has the characteristics of both of the previous two types and is mostly seen in young people. Generally, smaller moles can be treated with laser and ionization methods, but there is a certain possibility of recurrence. However, there is a certain possibility of recurrence. Moreover, laser treatment is not able to leave a pathologic examination for a clear diagnosis. Generally speaking, if the nevus is slightly larger or more active, it is better to consider surgical excision and leave pathology for clear diagnosis. Some people are concerned about scarring. After laser treatment, scabs will be formed, and there will be a slight indentation after the scabs fall off, which will gradually flatten out in 3-6 months. Surgical excision is mostly done by shuttle excision, where the incision will be a little longer than the diameter of the mole, usually in the shape of a line. We can imagine that a nearly circular excision, with sutures, would have small bumps at the ends, so we choose the shuttle shape to be flatter. Most plastic and cosmetic surgeons choose their incisions based on the direction of the natural textural folds of the skin, and they are fine sutures with small stitches. Trusting the expertise of the plastic surgeon, the scar will usually be less noticeable after the recovery period, depending on the surgeon who performed the surgery for you. What is at high risk of malignancy? Short-term rapid growth, thickening, protrusion, or the original dark color becomes lighter, the original light becomes darker, the original uniform color becomes uneven. Several very small moles appear around one mole, like satellites circling the earth, which we call satellite lesions. There are also moles that have not changed before that suddenly break out, or repeatedly break out and are not easy to heal. There are also places that are easily irritated by shaving for boys, eyebrow trimming and hair combing for women. Places where the mucous membrane of the lips and mouth is easily irritated by spicy, hot and cold foods, and places where the mucous membrane of the perineum is easily irritated are recommended to be dealt with as early as possible.
