In children with elbow dislocation and elbow fracture, activity exercises are required after removal of external or internal fixation, otherwise, elbow extension, flexion and rotation may be limited, affecting elbow joint function. Generally speaking, elbow dislocations are repositioned and external fixation is removed after 2-3 weeks of external fixation and activity exercises are performed immediately; elbow fractures, either by external fixation with manipulation or by internal fixation (including steel pins left in the skin or outside the skin), are started with elbow joint activity exercises in 3-6 weeks (depending on individual differences). Because of the pain associated with early training, children generally have a fear of pain, poor self-control and poor cooperation, so it is important to explain this to the child in advance and seek the child’s cooperation, i.e., psychotherapy. Reasonable arrangements should be made for the child to participate in daily activities, using the elbow joint instead of the affected limb, such as encouraging the child to use the affected hand to hold objects, shoot balls, and pass things. Parents or non-professionals should avoid helping the child to move passively without professional guidance to avoid secondary fractures or local congestion and edema due to strenuous activities, formation of ossifying muscle inflammation, and impaired joint movement or even ankylosis. Elbow joint straightening exercises Sit the child in a sitting position, place the affected elbow on the table with a towel under the elbow, hold an apple or an object of interest to the child with the palm of the hand facing upward, and stretch the elbow joint to the maximum by relaxing the muscles, 10 each time, 3 times a day in the morning, midday and evening, requiring the straightening amplitude to increase gradually from one time to another. Flexion of the elbow joint exercises The child is seated, the distal dorsal side of the affected forearm is placed against the edge of the table, a towel is placed on the distal dorsal side of the forearm, the palm of the hand is turned toward oneself, and the elbow joint is flexed forward using the forward leaning movement of the body, again 10 times each time, 3 times a day, morning, noon and night, with the amplitude of flexion gradually increasing one at a time. Rotation of the elbow joint exercises The child takes a sitting position, bending the elbow 90 °, the child’s forearms flat on the table, both forearms under the towel, hands clenched thumbs facing up for the neutral position, thumb rotation towards the inside for the rotation of the front, thumb rotation towards the outside for the rotation of the back, the same 10 each time, a total of 3 times a day in the morning, midday and evening, the rotation amplitude gradually increased from one time to another. Be careful not to rush, as long as there is a little progress every day, most of the elbow joint function can be restored to normal.
