Both can manifest as blue or greenish grey pigmented spots, but Mongolian spots are mostly found on the lumbosacral and buttock areas and mostly fade with time, whereas nevus of Ota is mostly found on the face and around the eyes and can spread to areas such as the eyelids and rarely fades.
