I. Initial consultation
Ms. Sun, 67, came to our hospital because of the floating black shadow in front of her eyes. She said that she felt very uncomfortable because of the dark shadows floating in front of her eyes, which also affected her housework. She had never had any problems with her eyes before and her vision had always been very good, but now this symptom suddenly appeared and she was afraid that her eyes would go blind. After observing Ms. Sun’s eyes, I told her that the situation was not serious, and combined with the symptoms mentioned by the patient, I suspected vitreous turbidity.
II. Treatment process
Since vitreous turbidity was considered by physical examination and clinical manifestations, I asked the patient to undergo an eye examination first to clarify the specific situation of her eyes. Ultrasound examination revealed a moderate amount of dotted echogenicity in the mid vitreous of both eyes, and mild posterior polar spherical wall echogenicity in both eyes, clearly indicating vitreous opacities with mild posterior scleral staphyloma. I then told the patient that this was a physiological vitreous opacity, which usually did not affect her vision, but it was difficult to restore her to her original appearance. Ms. Sun was given oral lecithin complex iodine and aminopeptide drops to improve the symptoms of dark shadows in front of the eyes and reduce the impact on daily life, and she needed to be seen again in 1 month. Ms. Sun agreed to the medication and went home to recuperate after taking the medication.
 
III. Treatment effect
When the patient came to the outpatient clinic for follow-up after 1 month of medication, she reported that the dark shadows in front of her eyes were reduced because she insisted on taking oral lecithin complex iodine tablets and aminopeptide eye drops every day, indicating that her symptoms had improved. The patient was advised to continue to take the medication to avoid an increase in the area and number of black shadows, and to seek medical attention promptly if discomfort occurs.
IV. Notes
We are glad that Ms. Sun’s vitreous turbidity symptoms have been relieved, but we still need to advise the patient to take more rest in daily life, avoid excessive use of eyes, do not stay up late, and get enough sleep, which will help to improve the symptoms; in the daily diet, we can eat more iodine-containing foods, as well as other meat, eggs, milk, cereals, fruits and vegetables, etc., to maintain a balanced and comprehensive nutrition, which is also beneficial to the relief of symptoms; in addition, we also need to go to In addition, regular checkups at the ophthalmology department are needed to prevent other eye pathologies and to achieve early detection and treatment.
V. Personal insight
Vitreous opacities include physiological and pathological. Pathological vitreous clouding is mostly caused by eye trauma, inflammation or retinal lesions, which may affect vision if not treated in time. In this case, Ms. Sun is considered to have physiological vitreous clouding because there is no obvious cause and trauma, which is not serious and usually does not affect vision, and mostly occurs in the elderly because of the gradual physiological liquefaction of the vitreous clouding, which usually cannot be completely cured due to the aging of the eye and can be devastating to the patient and requires family companionship and comfort.
