Surgical treatment of clavicle fracture

  The treatment of clavicle fracture: 1, the green branch fracture: mostly children, the non-displaced can be fixed with “8” bandage, for those with angular deformity, after reset, still maintain the alignment with “8” bandage. For larger children with a tendency to re-displacement, an “8” plaster bandage is appropriate.  2.Adults with non-displaced fractures: use “8” cast bandage for 6-8 weeks and pay attention to the shaping of the cast to prevent dislocation.  3.Displaced fracture
The operation is as follows: the patient is seated with the arms crossed, the chest raised, the head tilted and the shoulders extended back. The operator stands behind the patient and holds the patient’s shoulders with both hands at the anterolateral side (or the lateral side of both elbows) and presses them upward to make them extend their chest, while using the anterior part of the knee to form a fulcrum behind the lower thoracic section of the patient, so that the fracture can be repositioned more ideally. On this basis, an “8” plaster bandage is applied. In order to avoid axillary vascular and nerve compression, during the whole process of wrapping the plaster bandage, the assistant should place both middle fingers and index fingers in a crossed position in the bilateral axilla of the patient in a squatting position. The plaster bandage is wrapped through the middle finger and index finger of both hands of the assistant and continues until the plaster bandage is formed. In general, clavicle fracture does not require complete anatomical alignment, and good function can be obtained after fracture healing as long as the displacement is not very serious.  4.Surgical treatment: The indications for surgical treatment include open fractures; fractures with combined vascular and nerve injury; displaced fractures of the outer end or outer 1/3 of the clavicle with rupture of the rostral clavicular ligament; fractures without connection. The internal fixation method can be chosen from “8” wire, Kirschner pin or plate screw fixation depending on the type and location of the fracture.
