Recently, the cardiothoracic surgery department of our hospital successfully performed multiple deformity radical treatment for a 56-year-old male patient with congenital heart disease, severe tetralogy of Fallot combined with severe gout, and the patient has successfully recovered and was discharged from the hospital today. It is understood that there are not many reports in China on the successful deformity radical treatment of severe tetralogy of Fallot at such an advanced age in adult congenital heart disease. The patient, surnamed Sun, who lives in Yinzhou, Ningbo, has had cyanosis of the lips since childhood, shortness of breath after activity, and severe gout 10 years ago. When he was young, he went to Shanghai and other major hospitals several times for treatment, but all said that the operation was complicated and difficult to cure. In mid-February this year, he developed persistent hemoptysis and came to our hospital for medical treatment. After admission, cardiac ultrasound showed congenital heart disease, tetralogy of Fallot, giant ventricular septal defect, aorta riding across the ventricular septum (50% riding rate), right ventricular hypertrophy, and severe stenosis of the right ventricular outflow tract and pulmonary artery trunk and left and right pulmonary artery openings (about 0.5 cm in diameter). Cardiac catheterization further confirmed the echocardiographic diagnosis. After intra-departmental discussion, Director Guofang Zhao concluded that the patient had a hope for surgical cure, but the right ventricular outflow tract and pulmonary artery stenosis were both severe and extensive, with abundant collateral circulation, which made the operation more difficult; her age and long-term history of gout increased the risk of surgery. After careful preoperative preparation and surgical plan design, on March 12, with the close cooperation of anesthesiologists Hu Xukai and Wu Yixiao and extracorporeal circulation doctors Dong Caijun, Zhao Guofang, Yang Minglei and other doctors successfully operated on Mr. Sun to repair the ventricular septal defect, correct the aortic span, resect the hypertrophic right ventricular outflow tract muscle crest bundle, cut and make a transvalvular patch to widen the narrowed right ventricular outflow tract and The main pulmonary artery trunk and the left and right pulmonary artery openings were widened. After close monitoring and careful treatment, Mr. Sun recovered smoothly, and his mouth and lips are red. All the indexes were satisfactory in the review and he was discharged from the hospital today. Among cyanotic congenital heart diseases, tetralogy of Fallot is the most common one. Without surgery, the mortality rate is 25% within 1 year of age, 95% within 40 years of age, and few live beyond 60 years of age. As surgical techniques have improved, the mortality rate for radical surgery for tetralogy of Fallot has decreased significantly, but the mortality rate for surgery in the adult group is still above 10%. In this patient, with such an advanced age, severe stenosis of the right ventricular outflow tract, main pulmonary artery, left and right pulmonary artery openings, and long-term gout and other high-risk factors, surgical survival has rarely been reported in the province.
