The course of treatment for infertility is long, usually 3 months for 1 course of treatment. This is based on the time required for the process of sperm production to maturity. We know that spermatozoa are produced by the varicocele of the testes, and in general sperm production goes through three stages. The first stage is the formation of larger primary spermatocytes from smaller infantile spermatogonia that undergo several divisions; the second stage is the division of primary spermatocytes to form two secondary spermatocytes, which undergo another mature division to form two spermatocytes; and the third stage is the development of spermatocytes into spermatozoa through a series of changes. However, the spermatozoa are still delicate and do not have the ability to move and fertilize. They have to stay in the epididymis (an accessory gonad in men) for some days so that they can have the ability to move and fertilize. The process of sperm production in the testicular varicocele takes about 74 days, after which it takes about 16 days to continue maturing in the epididymis, so that the process of producing a mature sperm takes about 90 days. Therefore, patients who receive medication should not be in a hurry, but must adhere to their treatment and not change doctors or treatment plans too often, and should not have their semen rechecked too often to verify the effectiveness of their treatment, especially for patients with oligospermia or azoospermia whose cause is in the testes.
