Four treatment methods of Chinese medicine for ear, nose and throat diseases

  The four-combination therapy of otorhinolaryngology is an original and new idea created by Professor Zhu Xiangcheng, a famous otorhinolaryngologist of Zhejiang College of Traditional Chinese Medicine, from his long-term clinical work, integrating the strengths of various schools of thought and his own clinical experience to inherit and develop the clinical treatment method of otorhinolaryngology in Chinese medicine, which opens up a brand-new way of thinking for us in the future and is worthy of further summary and promotion. The so-called four-combination therapy is to combine overall and local, internal and external treatment, treatment and psychology, and medicine and diet in the clinical treatment of each otorhinolaryngological disease under the guidance of the basic theory and holistic concept of Chinese medicine. We have achieved remarkable results in the treatment of otorhinolaryngological diseases through the four-combination therapy. Now I would like to give a rough introduction for the benefit of my colleagues. Due to the author’s incompetence and lack of understanding, it is inevitable to miss a lot of points.  In the treatment of otorhinolaryngological diseases, according to the relationship between the otorhinolaryngology and the meridians of the internal organs, he focuses on the overall identification of the internal organs and the overall regulation, and believes that the local lesions of the otorhinolaryngology are all manifestations of the deficiency of the internal organs. For example, in treating otorhinolaryngological diseases with kidney tonics, he grasped the theory of “kidney is the essence of the first heaven”. Deficiency and imbalance of kidney yin and kidney yang is one of the common pathological changes of otorhinolaryngology. According to the ancient saying, “Injuries to the five viscera must affect the kidneys”, he believes that all otorhinolaryngological diseases that damage the internal organs over time must affect the kidneys. Therefore, he starts from tonifying kidney yin and kidney yang to treat chronic diseases of otorhinolaryngology such as atrophic rhinitis, allergic rhinitis, chronic pharyngitis, chronic non-suppurative otitis media, and vagal effusion. Another example is that he proposes five methods for nasal obstruction of nasal diseases: dispelling evil and frustrating the surface, tonifying the deficiency and consolidating the health, raising the clear and lowering the turbidity, activating the blood to dispel blood stasis, and clearing the qi and lowering the fire to adjust the internal organs, yin and yang, qi and blood to dispel evil and clear the orifices. For pharyngeal disease hoarseness, it is believed that the cause of hoarseness can be summarized in three categories, one is caused by external causes, due to the attack of external wind evil. The second is due to internal causes, mainly the internal organs deficiency. The third is not internal and external causes, such as damage to the veins and channels (surgery) or blood and phlegm accumulation of pressure and obstruction of the meridians, or loud cry, eulogy injury throat. The treatment should be based on different etiology and pathology, and the main emphasis should be on identification and prescription of medicine in order to receive good results. In the treatment of violent deafness of ear disease, it is believed that when identifying the evidence, attention should be paid to the process of its onset and the changes of evil and positive. These theories, applied to clinical practice, are derived from proficiency in the basic theories of TCM, not forgetting the essence of TCM treatment – the holistic concept of diagnosis and treatment, mastering, applying flexibly, and giving full play to the therapeutic strengths.  In the treatment of otorhinolaryngological diseases, we emphasize the overall diagnosis, but we do not forget the local changes. We identify the various changes in the local form and color of the otorhinolaryngology, and conduct the identification of cold, heat and deficiency to provide an important element and basis for the overall diagnosis, and make the correct diagnosis and legislative prescription. At the same time, we pay attention to the external treatment of the local area, following the ancient teaching that “the physical body and the nine orifices of the tangible disease, where there is actually evil Qi condensation …… must use the external treatment method, can be healed by hand”, through the external treatment of the local area to achieve the overall treatment effect, but also to control or reduce local symptoms. Therefore, Professor Zhu Xiangcheng is also good at local external treatment and has rich clinical experience. In 1988, he hosted the first national training course on the preparation, clinical application and modern research of laryngology blowing medicine, which was valued by otorhinolaryngology workers nationwide. Through clinical practice, he summarized the “therapeutic effect of laryngology blowing medicine”, “the rule of laryngology blowing medicine”, “laryngology blowing medicine preparation, drug selection, processing and concoction” and other experience, and the He also conducts scientific research on the “clinical and pharmacological study of laryngeal dissipative medicine”. He believes that external treatment of otorhinolaryngological diseases can supplement the inadequacy of internal treatment, and that the overall treatment combined with local external treatment can achieve rapid results. However, the use of local external medicine, especially the blowing of the throat, should be the same as the internal treatment method, and must be based on the clinical symptoms of different changes, the identification of cold, heat and deficiency, the composition of external medicine and the amount of medicine should also be added and subtracted in order to get results. Such as with the same blowing prescription, according to the local changes in different conditions should be added or subtracted changes; can also use different nature of the blowing prescription, with different nature of the lesion; can also be due to different symptoms of local lesions in the throat, and choose the function of the main treatment of different blowing prescription, his clinical experience of these local external treatment are based on the overall dialectical concept of treatment, the succession of Chinese medicine laryngology blowing medicine, carry forward, clinical application and research has made great Efforts.  In the process of clinical diagnosis and treatment, he asks detailed medical history, carefully examines, carefully picks up, looks at the moss and pulse, identifies and treats, and prescribes medicine. However, we also attach great importance to the patient’s psychological activities. The changes of the seven emotions, such as fear, worry, anxiety, depression, sadness, excitement, anger, etc., can lead to the disorder of qi and blood in the internal organs and the dysfunction of various diseases. Emotion and will can cause and cure disease. Maintaining a good mental state and emotional changes are not only beneficial to health, but also enable the sick to recover. Especially in otorhinolaryngology, patients will have all kinds of doubts, although you have prescribed a good prescription, if you do not make a serious examination of the patient and make some introductions to the disease to remove unnecessary doubts, it will often produce the result that the prescription is right, but the effect is not obvious. Therefore, it is believed that the patient’s diagnosis and treatment must be combined with psychological treatment, fully mobilize the patient’s subjective initiative, correct understanding of the disease, establish confidence to overcome the disease, and maintain a good mental state, in order to receive twice the result with half the effort. He also stressed that this is an important element of Chinese medicine diagnosis, which has been valued by medical practitioners for generations, “Nei Jing” says: “People have five organs into five qi to produce happiness, anger, sadness and fear.” Like other disciplines, otorhinolaryngology often injures the internal organs due to abnormal changes in the patient’s emotions, resulting in dysregulation of the qi flow, disorders of qi-blood function, obstruction in the otorhinolaryngology, cerebral tinnitus and deafness, nasal bleeding, voice changes, etc. Patients are often suspicious of lesions in these areas, especially cancerous ones. Therefore, after careful and detailed investigation, we can exclude the lesions and use the medication according to the symptoms and give a reasonable explanation to the patient, remove unnecessary doubts and achieve the effect by combining with psychotherapy.  Combination of medication and diet In the clinical consultation, there is another feature that attaches great importance to the combination of medication and diet therapy, while prescribing medication, pay attention to dietary contraindications, considering diet therapy as a science and an indispensable part of modern comprehensive medical therapy. If the intake of food is not suitable for the needs of the body, it can disrupt the normal function of the organism and produce disease. In the case of disease some food intake will have beneficial effects on the sick body, while others will be harmful and aggravate the condition. As the Su Wen says: “eat more salt, the pulse is frozen weeping and change color; eat more bitter, the skin haggard and hair plucking; eat more pungent, the tendons are urgent and claws withered; eat more acid, the meat is callous and lips uncovered; eat more sweet, the bone pain and hair fall.” The “Suwen Wu Chang Zheng Da Lun” says: “big poison cure, ten to its six; regular poison to the disease, ten to its seven; small poison cure, ten to its eight; non-toxic cure, ten to its nine; grain, meat, fruit and vegetables to eat and nourish all of them, no make over, hurt its positive.” The “Golden Killing” says: “the taste of food, there are appropriate with the disease, there are harmful to the body, if appropriate to benefit the body, harm will become disease.” In the book “The Heart of Medicine”, it is said that “if food and drink are inappropriate, disease will rise. External evil take advantage of this, entanglement has been, immersion in the meridians, condensation couples, change the symptoms of a hundred ends, can not be better remember. The “Thousand Gold Formula” says: “The doctor must first understand the source of the disease, know its offense, to food treatment, food treatment does not heal, and then ordered medicine.” And also said: “Food can drain the evil and the internal organs, pleasing the mind and refreshing the spirit, to capitalize on the blood. If you can use food to calm the disease, release the emotion of the disease, can be considered a good worker.” According to these theories and his own clinical experience, he treats each patient according to their different conditions and physical attributes, and always instructs patients to pay attention to their diet, what they should eat and what they should avoid, and to “eat in time” in their daily life. In daily life, it is necessary to “eat with time”, “taste is not precious”, “all food is not thick taste, no strong taste heavy wine”, admonishing patients, which is very popular among patients.
