How to treat pancreatic cancer?

Cardia cancer is a common malignant tumor of the digestive tract. It generally refers to cancer whose center is within 2cm above and below the gastroesophageal junction. Cardia cancer tends to have low differentiation and high malignancy. The main points of diagnosis are progressive difficulty in eating, pain behind the sternum or burning sensation in the upper abdomen, emaciation and anemia. Diagnosis can be confirmed by X-ray examination, which shows broken mucosal folds and limited stiffness of the cardia at the end of the esophagus, filling defects, and luminal narrowing or obstruction in advanced stages. Esophagoscopy can detect the lesion, and biopsy can be taken to find cancer cells.

Treatment methods: 1.Radical pancreatic cancer surgery.

2.Esophageal bypass surgery.

3.Chemotherapy or local radiotherapy.

4.Medication: apply traditional Chinese medicine and anti-cancer drugs to relieve the symptoms of advanced patients.

When cardia cancer invades the gastric body, our department performs radical pancreatic cancer surgery through the abdomen. Since the main lymph node metastasis of cardia cancer is downward-oriented, so for the tumor in the cardia, as long as the lower esophagus is not involved in the selection of transabdominal incision, it can fully expose, not only can the abdominal lymph nodes be completely cleared, but also the lymph nodes next to the esophageal cleft and the corresponding regional lymph node clearance, which is conducive to improving long-term tumor-free survival.

In our department, the method of GI reconstruction after total gastrectomy mostly adopts the free jejunal three-chamber gastric substitution method to improve the efficacy. Advantages: 1. The wide gastric pouch can store and absorb food.

2. The normal food operation channel is maintained, and food and biliopancreatic fluid can be mixed in the gastric pouch.

3.It helps digestion and absorption without postoperative sequelae such as reflux esophagitis, dumping syndrome and diarrhea. The project has won the Jiangxi Provincial Science and Technology Progress Award.
