The prevalence of congenital myotonic neck in children is 0.42% to 1.3% as reported in the domestic literature. The pathogenesis of the disease is still in disagreement in the academic community. There are a few recognized theories including: ① It is believed to be related to injury. The sternocleidomastoid muscle on one side is injured and bleeding during delivery due to the birth canal or forceps compression, and the hematoma is mechanized to form a contracture. ②It is believed to be related to fetal position. The head of the fetus in the uterus is often tilted to one side, or it is related to the abnormal development of the embryo, but not to the process of delivery. (3) It is believed that the fetal head is not properly positioned during delivery, which obstructs the blood supply of the sternocleidomastoid muscle on one side and causes ischemic changes in this muscle. For the treatment of pediatric myotonic levator, surgery is mainly adopted abroad, and the timing of surgery is appropriate from 2 to 12 years old; domestic Chinese medicine mainly adopts Tui Na treatment, and the healing rate is more than 80%. As an important part of Tui-Na specialty, Prof. Wang Jin-Gui, on the basis of the unique techniques of traditional pediatric Tui-Na schools and after more than 30 years of clinical practice, summed up the special Tui-Na technique of “activating blood and regulating tendons” to treat pediatric myxedema. For the mass type of pediatric myelomeningocele: the treatment principle is to soften the hardness, reduce the swelling and disperse the knots, and adopt the techniques of kneading, pinching and sliding. ①The child lies on his back and exposes the skin of the neck on the affected side. The doctor sits on the side of the bed or on the opposite side of the child, holds the child’s neck with one hand, makes its back stretch, and kneads the sternocleidomastoid muscle with the other hand from the mastoid part of the sternocleidomastoid muscle with the finger kneading method, repeatedly kneading along its contour from top to bottom and increasing the kneading force around the mass, the operation time is about 5 min. The technique should not be too heavy and should be alternated with kneading to avoid the child crying due to pain. ③Subsequent to the anterior position, the doctor fixes the child’s head with one hand and takes the affected sternocleidomastoid muscle with the other hand, focusing on the mass, repeatedly operating 1 to 2 times. ④In the relay position, one hand holds the affected shoulder, the other hand holds the top of the child’s head and gradually tilts the child’s head toward the healthy side of the shoulder, repeatedly for more than 10 times. ⑤ The child is lying prone, and the physician is located on the affected side. Knead the oblique muscles on the affected side, and finish the operation by taking Fengchi and shoulder well. For non-lumpy type of pediatric myxomatous oblique neck: the treatment is to relax the tendons and detain the affected muscles, using the techniques of kneading, pinching and stretching. ① The child is lying on his back and the doctor is standing on the affected side. The doctor fixes the child’s head with one hand, takes and slightly raises the affected sternocleidomastoid muscle with the other hand, and repeatedly operates 2 to 3 times. In the secondary position, the doctor holds the shoulder on the affected side with one hand and presses the shoulder slightly downward, and supports the head on the affected side with the other hand, slowly pushes the child’s head to the healthy side, so that the child’s head makes passive lateral movements in the frontal plane, repeatedly for several times. ③ Following the former position, the doctor holds the child’s head and neck with one hand and rubs the sternocleidomastoid muscle on the affected side with the other hand. Finish the operation. On the basis of the above massage treatment, you can also use the cream massage method and local heat compress method. The application of the cream massage method and local hot compress method can effectively release the local muscle spasm of the child, eliminate the stasis and disperse the knots, move the Qi and blood, relax the tendons and activate the collaterals, and restore the function of the neck movement, so as to enhance the effect of the treatment. At the same time, the local hot compress formula includes safflower to invigorate blood stasis, regulate qi and open the channels; mannite to soften the hardness and disperse the knots; muxiang and sumac to move qi and disperse the knots to relieve pain; papaya and elongated tendon grass to relax the tendons and activate the channels. The combination of these methods can invigorate blood circulation and resolve blood stasis, soften hardness and disperse knots, soothe tendons and channels, eliminate stasis and relieve pain.
