What to do if you are “allergic” to sperm?

  Infertility is a common clinical condition in both male and gynecology. However, the causes are complex and there are many types of infertility. Now we will share with you the basic knowledge of immune infertility. Before talking about immune infertility, let’s talk about the two words “合” and “和”. The former is a mouthful of people and the latter is a mouthful of people. The Chinese characters, as the name implies, have a profound connotation. Let’s take a look at them together.  Immunological infertility is actually an allergic reaction, just like an allergy to a certain drug, food, pollen, sunlight, taste, etc. When you first come into contact with these things, your reaction may be relatively small and your body produces antibodies that are resistant to the allergic item. The next time you are exposed to such an item, the reaction is quicker and more violent than the first attack. The same is true for immune infertility, which is simply an “allergy” to sperm. Both men and women can be “allergic” to sperm. In the case of men, sperm allergy is like an autoimmune reaction, in which sperm penetrates the bloodstream in a certain way and stimulates an immune response in the blood, producing anti-sperm antibodies. In the case of women, the same thing happens when they are allergic to sperm from men, and they have a similar “allergic reaction” and produce anti-sperm antibodies that prevent the sperm from binding to the egg.  At this point, you have a general understanding of immune infertility. In short, it is a pathological reaction similar to an allergic reaction that prevents the normal process of conception. Looking back at the words “and” and “together”, it can be said that immune infertility is a situation in which the couple is “together but not in harmony”, the first one being the normal sexual life of the couple – the process of intercourse. The first one refers to the normal sexual life of the couple – the process of intercourse – and the normal sexual life. The latter one is the metaphysical one, which refers to the union of sperm and egg, which is equivalent to the Chinese medical term “God”. The Yellow Emperor’s Classic of Internal Medicine says: the mother is the foundation, the father is the tattoo; those who lose their gods die, those who gain them are born. The couple is “together” in physical form, but cannot achieve “harmony” at the level of the gods, that is, they can open and close, but not in harmony.  Western medical treatment: 1) Etiological treatment Protect the function of blood-testis barrier and reduce autoimmune reaction. Anti-inflammatory treatment for inflammation of testes, epididymis and secondary gonads can make the antibody titer decrease; surgical removal of unilateral lesioned testes, epididymis or unblocking of obstructed ducts.  2) Reduce antibody titer and improve fertility. For women with sperm agglutinating antibodies in their bodies, the male partner should use a penis condom during sexual intercourse to avoid contact with semen. After 6 to 18 months, 40% to 60% of women’s antibody titers have decreased, and conception can be expected during ovulation; immunosuppressive therapy can also be used under the guidance of a doctor. Men can apply testosterone to inhibit sperm production in order to lower the antibody titer, but it can only have a temporary effect, and they should strive to conceive when the spermatogenic function is restored after stopping the drug and the antibody is still at a low level.  3) Chinese medicine treatment has better efficacy. It has been reported that the use of heat detoxification, blood circulation and blood stasis or nourishing Yin and lowering fire can achieve very good results, but the basic principle should be discriminatory treatment, the effect is good.  4) Perform artificial insemination. In conclusion, immune infertility can be prevented to a certain extent, including early correction of developmental abnormalities, active treatment of reproductive tract infections, prevention of reproductive tract injuries and attention to menstrual hygiene, etc.  TCM treatment: TCM believes that anti-sperm antibodies in either the male or female partner are an overreaction of immunity and belong to a kind of allergic constitution. Western medicine uses immunosuppressant treatment, while Chinese medicine uses nourishing yin, removing dampness, resolving phlegm, clearing heat, resolving stasis, tonifying the spleen and the spirit, etc., to correct the over- or under-reaction of the immune response and regulate the disorder of the immune response. It is worth mentioning that “allergy” is considered to be related to wind in Chinese medicine, and immune infertility is also a kind of “allergic reaction” in human body, so allergy decoction and other methods can be used in the treatment of immune infertility. Immune-boosting agents are needed for people with low immune function. Generally speaking, the immune booster is the Chinese medicine of strengthening the spleen and tonifying the kidney. For those with immune dysfunction, immunomodulators are needed, that is, sparing the liver and dispersing nodes. Those with hyper-immune function or over-immune response need to be treated with immunosuppressants, which means strengthening the spleen and removing dampness, clearing heat and phlegm, and activating blood and water.  2.
