What are the main diseases seen in male medicine?

Male outpatient clinics mainly see the following types of diseases.
  1. Infertility.
That is, those couples who have been married for a period of time and cannot conceive can come to the male outpatient clinic. We know that conceiving and raising offspring is a slap in the face. It is not only related to the fertility of women, but also closely related to the fertility of men.
In addition, it is necessary to be clear as a patient. Some preliminary tests for male infertility, such as physical examination, semen examination, hormone examination, etc., are non-invasive. It is easier and quicker than the female infertility test, so if you encounter problems in this area, you may want to go to the male clinic of a regular hospital first.
  2, sexual dysfunction.
Common sexual dysfunction are: erectile dysfunction, premature ejaculation, seminal emission, abnormal erection, etc.
Men encounter impotence, premature ejaculation of this type of problem is often shy to speak, which is a normal psychological. But if not treated in a timely manner will add to the psychological burden and destroy the relationship between husband and wife. This is why some patients are often victimized by some bad advertisements and mistakenly take the so-called health products. The actual fact is that you should go to a male specialist in a regular hospital for this kind of situation.
The ejaculation of teenagers is a normal physiological phenomenon, but it is not normal to have too many ejaculations or to have a psychological burden due to ejaculation. If you’ve come across such a situation, you may want to visit a male clinic to see if it’s all just a result of your poor sleeping posture.
  3, prostate disease.
Chronic prostatitis is a common and prevalent disease in the male outpatient clinic. It has complex symptoms and lacks specificity. This disease often makes patients very distressed. Benign prostatic hyperplasia, also called prostate hypertrophy. It is the privilege of the elderly. Men tend to develop prostate enlargement as they get older. The main symptom of it is the difficulty in urinating and the increase in the number of nocturnal urination, which in layman’s terms means that you have to wait to urinate, effort, not far to urinate, and get up more often at night.
  4, psychosexual abnormalities.
This kind of disease mainly has hypersexuality, hypoactive sexual desire, sexual aversion, etc. Usually this type of disease performance is very obvious, but fewer people visit the clinic. Often, these diseases are ignored because of the taboo against sex.
Of course, the male clinic also sees other patients, such as: orchitis, epididymitis, male menopausal diseases, and sexually transmitted diseases related to men.
Tips on how to see a male doctor
Nowadays, we are all afraid of being ripped off: buying something by a business, eating something by a restaurant, seeing a doctor by a doctor, especially liver disease, male disease, etc. There are too many advertisements and it is difficult to distinguish the real from the fake. So as a patient how should you choose a hospital and doctor?
  1.Are you sick?
You may think this paragraph is purely superfluous. If I am not sick, why should I see a doctor? But the fact is that a small percentage of outpatients are not sick. For example, people often ask, “Is my penis too short? The truth is that the vast majority of people do not have penises of the length of the male stars in those adult videos. If you are married and can complete sexual intercourse normally, why worry about being short and small? The sexual pleasure a woman can get is not proportional to the length of her penis. Also some overly sensitive gay men tend to think they are impotent or prematurely ejaculated because of one or two losses such as being prematurely ejaculated or the lack of erectile hardness. In fact, such a situation does not become a diagnosis of impotence and premature ejaculation, and is often related to your temporary increase in work pressure and the impact of life events. At this time, you just need to communicate with the doctor appropriately to solve the problem, do not consult a charlatan or believe in bad advertisements.
2.What kind of hospital should I look for?
There are too many advertisements, and there are big hospitals and small hospitals too. Which one should I go to? This question is not very clear. Nowadays, with the development of society, advertising is a normal way to expand the influence of hospitals. However, there are some hospitals that make some unrealistic propaganda in their advertisements to lure patients to visit them, which is quite unwarranted. So how to choose a hospital? We think there are several ways.
  (1) Try to choose a larger public hospital, and avoid smaller private hospitals or small clinics. This is especially important for first-time patients. Because the first few visits are often the time to clarify the diagnosis. The time to determine the treatment plan. If you go into a black clinic at this time, it will have a very bad impact on your follow-up treatment.
  (2) Look at the brand of the hospital, generally the more formal hospitals are mostly public hospitals. In addition, clinical experience and scientific research are two inseparable conditions for the development of medicine. Therefore, the larger hospitals are often affiliated with medical schools or teaching hospitals. These hospitals often have some teachers from the medical school to see patients, and doctors from the hospital will also visit the school to give lectures.
  (3) Pay attention to the communication with other patients, so as to know the overall reputation of the hospital.
3, what kind of doctor should I look for?
Nowadays, there are so many professors and experts in male clinics that it is dazzling. In fact, in addition to having a wealth of clinical experience, a medical expert must first be a scientific worker, understand the scientific method of conducting medical work and research, and know the latest advances in medicine. At the same time, a medical specialist must also have good medical ethics and a passion for solving the problems of the patients. It is undeniable that most of the specialist professors in Shanghai meet this standard. However, it must be admitted that there is a growing proliferation of specialist clinics, and many so-called specialists are still far from the above requirements.
As far as many clinics and hospitals are concerned, a large number of experts and professors are actually self-appointed. After being packaged by some clinics and hospitals, on the surface, they are indeed a powerful lineup. But upon closer examination, it turns out that their biggest selling point is mostly their years of experience (regardless of what level of hospital they are in). Some of the urological male experts have not been trained in this area at all, but only engaged in general medical work for many years, and then listed as male experts, their medical quality can not be guaranteed. Such a specialist clinic is very deceptive. It is only after the patient has lost money and energy that he or she may be able to see the true colors. Therefore, patients are advised not to be greedy for convenience and believe in the advertised propaganda. Professor of fact is a senior teaching title in China, and a professor of medicine is necessarily an employee of a medical school, a scientific institution and its subordinate units. The term specialist, on the other hand, is very unclear and is often used by black clinics. So when you can’t distinguish these titles, you may want to find the right hospital first, and then find a doctor.
  4.Don’t ask for Viagra
We have encountered some patients in the clinic work asked the doctor: can give open box Viagra. In fact, this situation is the most undesirable. The most important thing to see a doctor is not to take drugs but to diagnose, if it is not clear what the disease is to start taking drugs, then what do you want the doctor to do? Therefore, you must try to communicate with your doctor during the visit to determine a treatment plan that is suitable for your condition. Some patients are concerned that the medication prescribed by the doctor is more than they can afford due to financial reasons. If you encounter such a situation, you should speak clearly with the doctor you are seeing and do not refuse to speak, but take the initiative to ask the doctor to give you the most suitable treatment plan.
  5.Please keep the medical records
A medical record is a medical file. Properly trained doctors should pay great attention to the writing of medical records. It is an important basis for recording your visit and treatment. It can provide a lot of information to the doctor who sees you. Even if you are a regular patient of a certain doctor, he cannot remember all your medical records. Therefore, it is important that you bring your previous medical records with you when you receive treatment. This is especially true if you are changing doctors. On the other hand, if the doctor who saw you did not write your medical records carefully. You need to think carefully about the credibility of this doctor and hospital.
  Am I considered sterile?
Patient: My wife and I have been married for over a year and have been without contraception. My wife has never been able to conceive. Are we having infertility.
Answer: It is not easy to say. The World Health Organization defines infertility as a couple, living together, having a normal sex life, without contraception for more than 12 months, and the woman is not pregnant. Male infertility can be called male infertility if it is caused by the male partner.
We also need to take a closer look at the following aspects.
1, they are not living apart: if it is said to be living apart, or in this more than a year part of the time is separated from each other should be subtracted accordingly. Of course, the time of cohabitation before marriage should also be taken into account.
2, sex life is not normal: due to some special reasons, some couples living in a room together may not live normally. This is mainly manifested in the reduction of the number of times, the lack of understanding of sex, etc.
3, the age of the problem: age is not a necessary condition for the diagnosis of infertility, but for the actual clinical treatment must still be considered. Male fertility is less affected by age, but female fertility declines rapidly after the age of 35. Therefore, if the wife is older, the husband should have fertility counseling earlier so that he can be prepared as early as possible and solve the problem as soon as it is discovered.
4, marital history: If any party’s marital history is helpful in the diagnosis and treatment of infertility.
Of course, during the actual consultation, the doctor will consider other issues in order to determine whether you need to continue to wait, or medication, etc.. Such issues include the results of your physical examination (whether you have any significant fertility disorders), your occupation (whether you have been exposed to substances that are harmful to sperm production), your hobbies (whether you are a heavy smoker or alcoholic), your and your lover’s family history (whether you have siblings with similar conditions, whether your parents are close relatives), and a history of related diseases (such as urinary tract infections).
Is infertility my fault or my wife’s? Who should be tested first?
Infertility can be caused by both the male and female partners, as conception is a joint effort between the couple. Of course, it is also possible that both partners have fertility problems. Is it the husband’s cause or the wife’s cause? That depends on the results of the examination and diagnosis.
So who should be examined and diagnosed first? Since both partners may be the cause of infertility, men and women should be examined and treated in no particular order. Of course, since semen analysis is a non-invasive and convenient diagnostic method for the most basic assessment of male fertility, we mostly recommend that the male partner may wish to check the quality of semen first. In clinical practice, some patients come to have their semen routinely checked only when their wives have already settled all aspects of the problem, which is highly undesirable.
Some advice for infertile couples: Infertile couples must not be too depressed
The Chinese tend to have some of these traditional ideas: more sons, more blessings, passing on the family, no filial piety, no descendants, etc. We will not comment on whether these ideas, which have been passed down to us and rule us, are good or bad, good or bad. The problem of infertility does bring distress to many families.
We find that many patients suffer from such distress, and even this negative emotion is very serious in the process of seeing patients. As male physicians, we need to remind our patients that emotional well-being can have a significant impact on your fertility. For women, this often manifests itself in abnormal menstrual cycles and ovulation disorders. For men, it can manifest itself as a decrease in the quality of semen. Therefore, when you encounter the problem of infertility, you may want to treat it with a peaceful attitude, more communication with your doctor and confidence in overcoming infertility will often help you to solve the problem.
Why not bring your wife to the clinic?
For patients with infertility, we recommend that it is best for both partners to come together. The reason for this is simple: conception is the result of a joint effort by both partners. For patients with infertility, doctors often need to provide some guidance on fertility. In this case, the presence of both spouses is often very effective. This is especially important for first-time patients.
Of course, if your spouse is unable to come, please bring a more complete medical history with you.
Similarly, for patients with erectile dysfunction and premature ejaculation, it is best to have both partners present when determining the treatment plan.
How many days should I abstain from semen testing?
Human sperm are constantly being produced, and normal men can find a large number of sperm in their semen even after a relatively short period of sperm extraction after sexual intercourse. However, it is obvious that the quantity and quality of semen contained in semen at this time is not as good as the previous one. In order to be able to standardize the criteria and to take into account the influence of the time of semen extraction on the quality of semen, we generally have patients abstain from sex for a period of time before semen collection.
The duration of abstinence should be 2 to 7 days, preferably within 3 to 5 days. The number of days of abstinence should be counted from the last ejaculation (either sexual intercourse or seminal emission or masturbation). The length of abstinence will mainly affect the volume of semen and sperm density, while it will have less impact on sperm morphology and motility.
If the abstinence time is too short, the volume of semen tends to be low and the sperm density is also low. However, if the abstinence time is too long, the dead sperm rate may increase due to the prolonged storage time of sperm in the epididymis, which also does not reflect the normal situation. Abstinence for 3-5 days is basically in line with the sexual life pattern of most people, and the semen test results are closer to the physiological situation of the person.
Of course there are times when we may ask patients to abstain from sex for a longer period of time. This is mainly for those patients who have particularly low sperm density or whose sperm were not found in the last examination. If your doctor has no special requirements for your examination, the number of days of abstinence should of course be limited to 3 to 5 days.
Precautions for sperm collection and transportation
  1. You must abstain from sexual intercourse before collecting semen specimens, generally for 3 to 5 days (abstinence also includes no seminal emission or masturbation).
  2. The best way to collect semen is to have the patient come to the laboratory and have it collected by himself/herself by masturbation. Wash your hands and glans before collecting semen. Of course, if it is difficult to masturbate by yourself, you can do it with the help of your spouse. But remember that you can’t use latex or plastic condoms to collect semen because some of the lubricants in the condom may affect the quality of the sperm.
  3. When collecting semen, make sure that you collect all the ejaculated semen. Some patients may fail to collect all the semen due to emotional stress or negligence, which may lead to inaccurate test results. In normal ejaculation, the majority of sperm are found in the first 1/3 of the semen, so if they are lost, the test results may show oligospermia. Of course, the second 2/3 of the semen is also very important for human conception.
  4. The time taken to transport the specimen should not be too long, preferably not more than half an hour. At the same time, care should be taken to keep the temperature of the semen within 25-35°C during the transportation process. If the temperature during transportation is too high or too low, it may lead to inaccurate semen test results.
If there are any abnormalities during semen collection, you should promptly explain the situation to your doctor or the examining physician. Work with the physician to decide on the selection of the specimen. In addition, when collecting a semen specimen, the physician may ask the patient to do multiple tests. This is because our sperm production is sometimes highly variable and the doctor feels that he or she cannot make a judgment based on just one semen result. The generalist is not redundant to repeat the test. Such repeat tests should generally be performed 2-3 times at 1-2 week intervals.
Do I have to fast for the sex hormone test? What other issues should I be aware of?
Generally speaking, most of the tests that require blood sampling require fasting. Sex hormone testing is no exception to this rule.
The other thing to keep in mind is that the night before the test, patients should eat less fatty and high-protein foods and rest.
With regard to prolactin testing, it is recommended that you do the following before the test.
  1. Do not overeat a high-fat, high-protein diet and alcohol the night before the test.
  2. Try to maintain a normal mood.
  3. Take a short break in the hospital before the blood draw.
What are the symptoms of prostatitis
The symptoms of this disease can be summarized into four areas.
1, urinary symptoms: urinary frequency, urgency, pain, urethral burning, incomplete urination, etc.; or after urination and fecal exertion, there is a milky white liquid overflow; or there is a yellow spot stain on the underwear in the morning after waking up.
2.Pain symptoms: centered on the small abdomen and perineum, the pain is mainly swollen and dull, and may be accompanied by a feeling of cramping and heaviness.
3.Change of sexual function: sexual function may be reduced, enhanced or no change; it may be accompanied by seminal emission, premature ejaculation, painful ejaculation, etc.
4, mental and systemic symptoms: patients often have weakness, insomnia and dreaminess, forgetfulness, and even induce serious mental disorders.
In general, the symptoms of chronic prostatitis are very complex and lack specificity. The doctor also needs to take into account the specific condition to confirm the diagnosis. The patient should not take the above mentioned symptoms into account, but should assist the doctor in making a diagnosis on a case-by-case basis.
Why is it necessary to have a prostate fluid test?
The prostate is a chestnut-sized gland. It is located just behind the pubic symphysis (the bone behind our pubic hair) and in front of the rectum. Its back wall is about 4 or 5 centimeters from our anal opening. So the doctor can touch the patient’s prostate through the anus.
A gland like the prostate is called an exocrine gland in medical terms. It constantly secretes fluid into the cavities of our body. Since the prostate gland wraps around the urethra, the vesicles of the prostate gland naturally open to the urethra. When the prostate gland is diseased, the color, cell count, pH, and biochemical composition of the prostate fluid may change accordingly. This provides the doctor with “information” to diagnose the disease.
This is why doctors often obtain prostate fluid for diagnostic purposes by means of a prostate rectal exam.
How is prostate fluid collected? Do I have to abstain from sex? Does it hurt?
It is also important to abstain from sex for 3-7 days before collecting prostate fluid. Because prostate fluid is a major component of semen, if you have recently had sex, the collection may fail. In addition, ejaculation and emotional arousal can increase the white blood cell count of prostate fluid, which can affect the diagnosis. However, if you abstain from sex for more than 7 days, there will also be a build-up of white blood cells in the prostate, which can also create the illusion of inflammation.
When collecting prostate fluid, the patient urinates first. It is usually sufficient to bend over for the examination, with the hips elevated. Or take the right side lying position. When the doctor slowly inserts his finger through the anus and touches the prostate, the patient should open his mouth and relax the anus to cooperate with the doctor. The doctor uses massage to make prostatic fluid flow or drip out of the urethra and collect it with a glass slice or glass tube for testing.
Because our anorectal opening is very sensitive, patients may feel uncomfortable when having this test for the first time. However, no pain is usually felt. For patients with chronic prostatitis, some scholars recognize that extensive prostate massage is a better treatment.
The actual fact is that the doctor will not do prostate massage after the diagnosis of prostate tuberculosis or acute prostatitis, or if the prostate is in obvious pressure pain, to avoid the spread of inflammation or nodules.
How to look at the prostate fluid laboratory test
The normal prostate fluid lab results are as follows.
1, the appearance of milky white thin liquid
2, more lecithin vesicles, almost full field of view
3, a small number of epithelial cells
4, red blood cells can be seen occasionally
5, less than 10 white blood cells / high magnification field
6, PH value is generally between 6.2 and 6.7
  7. When the prostate is inflamed, the prostate fluid lecithin vesicles are reduced and have a tendency to accumulate in piles; the number of red blood cells may increase; the number of white blood cells exceeds 10 to 15 / high magnification field. In addition, the PH value of the prostate fluid will increase during inflammation.
What are the triggers for the onset of prostatitis?
  1. Irregular sexual life.
2, frequent sexual life or masturbation.
3, interrupted sexual intercourse or sexual intercourse for too long.
  4, long-term cycling, driving a long car.
5.Love to eat spicy food, alcohol abuse.
  6, long-term work tension.
7, too heavy, too much prostate massage.
  8, chronic constipation.
You can see that all of these factors can lead to prolonged prostate congestion, which may be their way to inflammation.
What are the signs of an enlarged prostate?
Prostate enlargement, also known as prostate enlargement, is a common disease in older men.
Our prostate wraps around the urethra, so when the prostate is enlarged it often causes difficulty in urination.
  The specific manifestations are.
1, increased frequency of urination, especially at night is most distressing to patients.
2, waiting for urine (when urinating, you have to wait a while to urinate)
3.Piss not far, easy to wet clothes and pants.
4.Urine is incomplete, and there is easy to have dripping without finishing.
  5.Hematuria may occur.
These symptoms in the symptoms of difficulty in urination will be aggravated in the case of cold, holding urine, overexertion, etc., and even urine retention may occur.
It may lead to diseases such as hemorrhoids, hernia, prolapse, and impaired kidney function.
Can prostate hyperplasia develop into prostate cancer?
Fifty percent of men over the age of 50 have symptoms of prostate enlargement; for men over the age of 80, the incidence of this condition is as high as 80 percent. Despite this high incidence, there is no causal link between prostate enlargement and prostate cancer, i.e. prostate enlargement does not turn into prostate cancer, but the two can co-exist. Prostate cancer is asymptomatic in the early stages, and when the tumor enlarges and presses on the urethra, it can easily be confused with prostate enlargement symptoms. In our country, 80% of patients are found to have distant metastatic lesions before prostate cancer is detected, losing the best time for treatment. Therefore, middle-aged and elderly men, once they find symptoms such as frequent urination, increased nocturia, poor urination and hematuria, they should go to a urological specialist hospital in time to distinguish whether it is prostate cancer or prostate enlargement as early as possible.
The first is that the treatment is not timely, the urinary disorder is considered a manifestation of aging, or it is difficult to talk about it, and only when the symptoms worsen do you go to the hospital, and as a result, you have to operate. The second is to treat the symptoms but not the root cause, some patients eat a period of medicine, urinary disorder relief, and then stop their own medicine. In fact, benign prostatic hyperplasia is a progressive disease that requires insistence on taking medication to shrink the volume of the prostate under the guidance of a professional doctor.
In addition, the middle-aged and elderly people with prostate disease should limit their water intake to about 1500 to 2000 ml a day, and try to force a clean bowel movement when urinating and defecating, and also do pelvic training, such as running, climbing, etc. Avoid playing mahjong or pedaling a bicycle for a long time.
