Repair of large bone defects in the lower leg – the miraculous effect of regeneration after bone lengthening

  Bone lengthening is the process by which a bone can be regenerated after it has been amputated.  Bone lengthening is not miraculous, so-called miraculous because the listeners are ordinary people who do not know much about mammalian physiology. What is really amazing is that the person who discovered this biological law is G.A. Ilizarov, an academician from the former Soviet Union, who discovered that any tissue in the human body can be regenerated by slow and rhythmic tension, which is called the theory of tension into tissue (bone).  In this paper, we report a typical case.  Male, 23 years old. He had an open fracture of the tibia, and after multiple surgeries, a large segment of the tibia was defective and locally scarred, but the skin coverage was still good after a flap graft had been performed on the anterior tibia. Due to the large tibial defect, the patient went from one doctor to another and suffered a lot. The doctors shook their heads and pushed their hands when they saw him, but finally I gave him admission. On examination, I saw that the soft tissue of the affected limb was in good condition, but there was shortening of the limb, about 3 cm, and there was an external brace on the lateral side of the lower leg. x-ray showed that there was a mid tibial defect of about 10 cm, with significant displacement of the distal and proximal segments, and severe osteoporosis.  In Dr. Kang Qinglin’s opinion, this is actually a very simple problem, which is not difficult for the so-called participants.  Treatment options: 1 Replace with a unilateral orbital external fixator and place it on the anterior medial side of the tibia.2 Align the distal and proximal breaks when installing the external fixation brace.3 Since there is more bone in the distal end, use distal to proximal distraction.4 Since the external brace needs to be worn for a long time and the osteoporosis is obvious, use hydroxyapatite-coated half nail fixation.5 Perform additional limb lengthening along with bone segment lengthening to restore limb length.  After 1 year, satisfactory results were finally achieved.  During lengthening, when the lengthened severed end met with the proximal host bone, local debridement was done without bone grafting.  The patient’s cooperation was greatly appreciated.  Illustration.
