How is infertility diagnosed and treated?

Causes: The three most common causes are Pelvic tubal factors 30-40%: such as tubal obstruction and fluid retention caused by infection and repeated abortions; pelvic adhesions; endometriosis, etc. Ovulation problems 25-30%: non-ovulation caused by endocrine disorders, or premature ovarian failure, etc. Male factors 30-40%: oligohydramnios or azoospermia. The quality of semen has been declining, so much so that this year the WHO revised the semen quality standard, for example, the density from 20* 10^ 6/ml to 15* 10^ 6/ml now. changes in lifestyle, increased work pressure and environmental factors all contribute to this phenomenon.  Infertility is often the result of multi-factorial influence of both husband and wife, so careful and comprehensive examination of both husband and wife is the key to treatment.  4.Clinical procedures Because of the increasing incidence of infertility and the poor knowledge of the people about infertility and reproductive health issues, the majority of primary medical units lack standardized training, technology and facilities for infertility diagnosis and treatment, so it leads to the proliferation of irregular medical practices for profit, resulting in multiple examinations, over-diagnosis and over-treatment.  Therefore, last year, led by Jiangsu Provincial People’s Hospital, our hospital participated in the standardized training on infertility and proposed a standardized clinical pathway for initial infertility screening: Step 1: semen analysis of the male partner, Step 2: gynecological examination of the female partner, ultrasound monitoring of ovulation, and necessary endocrine examination, and if no abnormality is found in the above 2 steps, a third step: tubal imaging.  With this step-by-step and targeted treatment, most infertility patients can successfully conceive a healthy baby. However, there are still 5-10% of patients who are unable to conceive successfully and can be assisted by the current advanced assisted reproductive technology. It is commonly known as IVF technology.  The concept of IVF is to remove the egg and sperm through certain means and then transfer the embryo to the mother’s uterus after it has reached a certain stage of development. Its success rate is 40-50%, and the success rate of our center is about 50%.  In conclusion, according to the World Health Organization, in the 21st century, infertility will become the third most common disease after tumors and cardiovascular diseases. Noteworthy On the one hand, the openness of modern life has led to an increase in premarital sex And the traditional Chinese education model is ashamed to talk about sex, so the awareness of self-protection is weak, lack of knowledge of sexual hygiene, repeated abortions, etc. lead to female pelvic inflammatory disease, tubal obstruction, which will inevitably lead to future infertility. On the other hand, social pressure is increasing, people are so tired of coping with heavy work and study that even childbirth has to be planned within a few months, so much so that overstress causes difficulties in conception and the quality of male semen keeps decreasing.
