Blood tests often show a decrease in white blood cell count, a relative increase in lymphocyte and monocyte counts, and an increase in white blood cell count and neutrophils when combined with bacterial infections. Most of the symptoms of viral colds are mild and self-limiting. In this case, nasal congestion, runny nose and sneezing with sore throat occurred after cold, and blood count showed an increase in monocyte count, and the patient’s symptoms improved after treatment with antiviral drugs.

II. Treatment history
At the time of the patient’s visit, physical examination: clear consciousness, poor mental health, obvious congestion in the pharyngeal cavity, bilateral enlarged tonsils of degree I, no pus adhesions, and no abnormal cardiopulmonary auscultation. Combined with the medical history and current clinical symptoms, we considered it to be a viral cold caused by cold, and performed routine blood + CRP examination, the results indicated that the white blood cell count was normal and the monocyte count was relatively increased. The common viral cold mainly manifested as nasal congestion, runny nose, sneezing, sore throat, cough, etc. Fever and chills were rarely observed, and symptomatic supportive treatment was generally the main focus. The patient was given Lianhua Qingfei capsule + Phenomethamine suspension, and was advised to take rest, eat lightly, avoid spicy and stimulating food, drink more water and keep indoor air circulation.
(Routine blood test in outpatient clinic)

Although the patient had a viral cold caused by the cold, he was treated by the clinic in time. After 5 days of outpatient drug treatment, the patient’s symptoms of nasal congestion, runny nose, sneezing, sore throat, cough, fever and chills improved significantly, his mental state improved, his appetite increased, and his bowel and stool were normal. The patient’s appetite increased and his urine and stool were normal. The repeat blood test showed no abnormality and the patient’s cold symptoms were completely relieved.
IV. Notes
We are glad that the patient’s cold symptoms are completely relieved after treatment. The common viral cold has more acute onset, is generally not contagious and is usually a self-limiting disease, and the routine use of antibiotics and antiviral drugs is not recommended. Patients should focus on prevention in their daily life, avoid strain and cold, pay attention to the combination of work and rest, enhance physical fitness, improve the body’s resistance, etc. During the high season of cold and flu, it is necessary to avoid crowd gathering, and for people with low immunity or basic diseases, preventive influenza vaccination can be given.
V. Personal insight
Viral colds are upper respiratory tract infections caused by viral invasion, including the common cold, viral pharyngitis, herpes pharyngitis, pharyngoconjunctivitis, etc. The infecting pathogens include respiratory syncytial virus, rhinovirus, adenovirus, coronavirus and influenza virus, parainfluenza virus, etc. The common viral cold mainly manifests as nasal congestion, runny nose, sneezing, sore throat, cough, etc. Fever and chills are rare, and complications such as otitis media, pneumonia and encephalitis occur less frequently. Blood count may be reduced, lymphocytes relatively increased, combined with bacterial infection, white blood cell count and neutrophils increased. Most of the symptoms of viral cold are mild and self-limiting, and the main focus is on symptomatic and supportive treatment. In this case, the patient improved from the cold through timely treatment, but protection should also be paid attention to during the season of high incidence of upper respiratory tract infection.
