Congenital preauricular fistula: This is a congenital condition. The main manifestation is the presence of a fistula in front of the ear, with occasional oozing and, if infected, painful redness and swelling. The incidence is about 1.2%, often unilateral, and usually not accompanied by other deformities. There is a small gray sunken opening in front of the foot of the ear wheel, which is usually uncomfortable, but there is often a grayish white scaly or cheese-like discharge from the fistula or extrusion. If there is a combination of infection, a localized abscess may form in front of the ear, with redness, swelling, pain, and pus overflowing from the fistula. A blunt-tipped probe can be used to investigate the course and depth of the fistula. Differential diagnosis In cases of deep fistulas complicated by infection, it is important to distinguish them from external auditory canal boils and postauricular abscesses, the main point of differentiation being that the former has a fistula followed by infection, while the latter has no such history. Treatment The disease is mainly treated externally to remove the lesion, but when the infection is acute, it can be treated internally and externally according to the canker (anti-infection, antipyretic agent, local dressing, etc.), and then surgical treatment is applied after the improvement. 1.Surgical treatment: Applicable to all types of fistulas. If the fistula is infected, first cut and drain the pus, and then surgically remove it after the acute inflammation is controlled. The fistula and the scar are then completely removed under local anesthesia to avoid recurrence. If the fistula recurs, the surgical excision is incomplete and a complete resection is needed. Children who do not cooperate need to be operated under general anesthesia. If, after the incision and drainage, the pus continues to flow, the inflammation is difficult to eliminate and cannot be removed surgically for the time being, then the Western medicine should be used to reduce the inflammation along with the Chinese medicine to treat the internal medicine and the local topical Chinese medicine.
