Many patients attach great importance to menstruation and believe that it must come normally in order to have the possibility of pregnancy, which is actually a misconception. The prerequisite for pregnancy is the normal ovulation of the ovaries, otherwise, even if the menstruation comes on time every month, if the ovaries do not ovulate normally, pregnancy is equally impossible. The most common anxiety is the lack of menstruation and the low volume of menstruation. It is believed that if you do not have a period or have a low menstrual flow, you are not ovulating and it is impossible to get pregnant. Some patients become anxious because their periods do not come for a long time, and even think that herbal medicine is ineffective because after taking herbal medicine for so long, they do not even get their periods. So they switch to progesterone, and after stopping the medication, they can get their periods, but the next time they are supposed to come, they don’t come again. The purpose of using herbal medicine is to restore ovarian function and promote ovulation, not to promote the onset of menstruation. If the goal is simply to bring on menstruation, then hormones must be the first choice. Undoubtedly, the use of hormones to promote menstruation has an immediate effect, but from the long-term effect, the use of hormones for a long time will actually inhibit ovarian function, which is why many patients use hormones to regulate menstruation for a long time, and the effect is ideal at the beginning, but for a long time there is no effect with hormones. In fact, it is the prolonged application of hormones that inhibits ovarian function. Similar to the ancient example of pulling up seedlings to help them grow, if the fruit on the tree is artificially ripened and picked before it is ripe, such fruit is inevitably not as high quality and tasty as the naturally ripe fruit. If you intervene artificially for a long time, it is possible that the fruit tree will never bear fruit again. I never use hormones in Western medicine to regulate menstruation for the same reason. A patient with polycystic ovary syndrome had a long period of amenorrhea and her basal body temperature was always low. I never used hormones to regulate menstruation, but I always added and subtracted medication based on the principle of tonifying the kidneys and strengthening the root, but I made her insist on taking basal body temperature. This case shows that as long as ovarian function is restored, pregnancy is possible even if menstruation has not returned to normal. Menstrual condition can reflect endocrine function to some extent, but it is not absolute. Therefore, I hope that you will not be overly anxious about menstrual abnormalities, because the fundamental goal of treatment is not to restore menstruation, but to restore ovarian function. In fact, once the ovarian function is restored, menstruation can naturally return to normal. Even if it is not normal, there is no need to worry too much. Another example is that I treated a patient with polycystic ovary syndrome who had prolonged menstrual cycle and very low menstrual flow, and the menstrual flow did not increase significantly after prolonged medication. This indicates that the ovarian function is gradually recovering until normal ovulation and pregnancy. The most important feature of TCM is the holistic concept and evidence-based treatment. Based on these two points, in the treatment of infertility diseases, we look at the whole person and consider in the long term, taking the fundamental principle of tonifying the kidney and strengthening the root, and taking advantage of the situation to restore the function naturally rather than intervening artificially. In fact, basal body temperature is quite important in the treatment of infertility. Basal body temperature is an important basis for dynamic observation of ovulation and luteal function as well as determination of early pregnancy. However, many people do not pay attention to basal body temperature, either they forget to take it, or they forget to bring it to the clinic, or they give up after a short time because they are afraid of trouble.
