Patient: Female, 37 years old. left neck mass found in January 2012, surgical biopsy in February, Hodgkin’s lymphoma (mixed cell type) stage 3 B diagnosed in March 2012. currently undergoing treatment at local hospital in Shandong. After consultation with Tianjin Hematology Hospital, chemotherapy with ABVD regimen was carried out in local hospital and whole body examination was performed after the 4th course of treatment. PETCT results: multiple small lymph nodes in the neck, 2*1*2.1 soft tissue density nodules in the anterior superior mediastinum, and no significant abnormal increase in FDG metabolism was seen as suppression of tumor activity after treatment. 1. Do the PETCT findings mean complete remission? How many more courses of chemotherapy do I need to do? 2. Is the mediastinal mass size from 4.9*2.7 (pre-treatment CT) to 2.5*1.9 (post-treatment CT after two courses) to 2.0*1.0*2.1 (post-treatment PET after four courses) considered fibrosis and does it create chemo-resistance? Can the later chemotherapy still work on it? Is it necessary to treat it with radiotherapy? 3.If I go to Shanghai for radiotherapy treatment, can I go to you for treatment advice? Doctor: Hodgkin’s lymphoma (mixed cell type) stage IIIB, treatment should be based on systemic chemotherapy, radiotherapy generally depends on the specific condition after chemotherapy, only for the large lesions or residual lesions before chemotherapy.
