Is surgery always required for submicrocephalic tonsillar herniation malformation?

Incidental imaging findings with asymptomatic or mild symptoms can be followed up with regular outpatient review and serial imaging, but if symptoms progress or MRI confirms progression of spinal cavity, early surgical treatment is required.
