The most common cause of shoulder pain is acromioclavicular impingement syndrome. Shoulder impingement syndrome does not refer to traumatic impingement, but rather to a chronic shoulder pain syndrome caused by repeated friction and impingement between the intra-articular structures and the rostral shoulder arch during shoulder supination and abduction activities. The osteophytes under the acromion make the gap between the acromion and the greater tuberosity of the humerus smaller. The patient’s repeated and excessive use of the affected limb, especially the movement of the affected limb over the head, causes the acromion and the greater tuberosity to collide, resulting in the soft tissue attached to the greater tuberosity being squeezed and rubbed. Abrasion of the tendon fibers over time can cause rupture of the tendon fibers, and if there is trauma, it may also cause instant rupture of the already worn tendon. Depending on the tissue involved, it can manifest as subacromial bursitis, supraspinatus, supraspinatus calcification, rotator cuff injury and biceps tendonitis. There are two main treatment methods: conservative treatment and surgical treatment. 1. Conservative treatment includes rest and avoidance of supination exercises; oral non-steroidal anti-inflammatory drugs such as Cilobal, physiotherapy; local closure, acupuncture, and tui-na may also be effective. Exercise appropriately, avoid trauma, pay attention to shoulder warmth, etc. 2.Surgical treatment: When conservative treatment does not work well, surgical treatment is needed. Surgery is mainly to remove the impingement factor, fully decompress, clean up the inflammatory tissue and suture the broken rotator cuff. Arthroscopic surgery is a minimally invasive procedure with small incisions, less pain, and less postoperative reaction. The surgery basically does not affect the muscular structure around the joint, so it is possible to exercise early after the surgery to prevent complications such as joint stiffness caused by long-term fixation of the joint.
