Is it better to cut or “spot” pigmented moles on the face and neck?

  In the clinic, we are often asked by patients: 1) Doctor, is it better to remove the mole on my face by surgery or by laser? 2. Is there no scar after laser “spotting”, but will there be a scar after surgery?  Just like the male patient in his thirties who just had surgery today, he also came to the clinic with these questions. He had a pigmented nevus of about 20px in diameter on the back of his nose, slightly elevated, with clear borders and round-like shape, with no obvious hairs on the surface, which was present at birth and did not grow outward significantly. My recommendation for him was to have them surgically removed.  Because the pigmented nevus on the back of his nose is relatively large, and if it is removed with laser or medicine, a small pit will be left in this location in the future, and when light shines on his face, this pit will become obvious due to uneven light reflection, especially when taking pictures. The human eye can easily catch this dark area of light, so it looks obvious from the visual perspective. The mark left after surgical excision is a line, and as time goes by, this line will become less and less obvious, because our skin has texture and folds, and plastic surgeons usually pay attention to the design of the incision to parallel the direction of the skin texture and folds, so in the future this line will be covered up by your skin texture and folds, which is equivalent to concealing from the visual perspective. Therefore, the line marks will be hidden from the visual perspective. Therefore, for pigmented nevi on the face and neck with a diameter of more than 3mm (about the size of a small grain of rice), it is recommended to opt for surgical treatment. For smaller pigmented nevi (less than the size of a grain of rice), after laser or drug “spot” removal, a “dot” will be formed with the contraction of the wound, and it will not look very obvious visually, so for this kind of pigmented nevi, both surgical excision and laser “spot” removal are possible. So for this kind of pigmented mole, both surgical excision and laser “spot” removal can be chosen. So if you have one large pigmented mole and several small scattered pigmented moles on your face, it is recommended to choose surgical excision to solve all of them at once, if there are only several small scattered pigmented moles on your face, then both methods can be chosen.  As for the question of whether the excision will leave a scar, human skin damage will leave a scar as long as it breaks through the dermis, which is a normal function of human self-repair and self-protection, and plastic surgeons cannot eliminate the scar, but can make it narrower, thinner and hidden through our unique techniques. For larger pigmented nevi, the depth of invasion will reach the dermis, and the dermis must be removed to completely solve the problem. The repeated stimulation is not good for the nevus, and if you want to remove the nevus completely, it will definitely leave a scar. As mentioned before, this pit-like scar will be more obvious than the linear scar.  In addition, the feature of our hospital’s fee is that the first pigmented mole is removed for 1030 RMB, and each subsequent pigmented mole is charged at 150 RMB. Some patients say, “I’ll cut one first to see how it works, and if it’s good then I’ll cut the other ones. Oh, you don’t need to take your own experiment, it has been proved in the clinic for thousands of times, so if you want to cut it, just cut it together.  Post-operative treatment: no bandages, 5-7 days to remove the stitches, the second day after the removal of the stitches can be wet, the first three months pay attention to sun protection. Before laser treatment and surgical removal of facial nevus (note the postoperative effect of each color arrow pointing to the nevus), the patient chose laser treatment first, the yellow arrow is the pit-like scar left after laser treatment, the visual angle is more obvious, the rest of the patient’s facial nevus chose surgical removal, the red and purple arrows are the almost inconspicuous line scar left after removal, the yellow arrow is the permanent pit-like scar left after the previous laser treatment .
