Acute chest pain is often present in myocardial infarction, and severe intense pain is often accompanied by gastrointestinal symptoms such as frequent nausea, vomiting and epigastric distention.
The onset is associated with stimulation of the vagus nerve by necrotic myocardium and reduced cardiac output and inadequate tissue perfusion. intestinal distention is also not uncommon and eructation may occur in severe cases.
