Older people have degenerative tendons and are more prone to injury, mostly occurring 2 to 6 cm above the Achilles tendon at the Achilles tuberosity attachment. 2.Open rupture can occur at any level, mostly caused by cutting when the Achilles tendon is tense, and the Achilles tendon section is neat. Performance There is a clear history of injury, the sound of rupture can be heard at the time of injury, open rupture with the presence of wound, local pain, weakness of the lower leg and difficulty in standing and walking. On examination, local tenderness, reduced plantarflexion of the foot, and depression of the Achilles tendon rupture can be palpated. It should be noted that when the Achilles tendon is ruptured and the tendon membrane is intact, the foot can still be plantarflexed due to the contraction of the posterior tibial, peroneal, foot [flexor and toe flexor muscles, but only the plantarflexion force is reduced. The patient was asked to stand upright with the heel off the ground, and the affected foot could not lift the heel, or the heel lifting force was weak. When the patient is lying prone with the feet hanging over the edge of the bed, the foot cannot be plantarflexed by pinching the calf triceps. Examination X-ray and ultrasonography may reveal discontinuous or blurred soft tissue shadows of the Achilles tendon. Treatment Fresh Achilles tendon rupture should be repaired by early surgery. A closed or sharp cut with a flush section can be closed with direct sutures using the Bunnell method. Tendonoplasty is appropriate for injuries with disjointed cauda equina. Old ruptures are generally repaired by kyphoplasty. After surgery, the knee should be flexed at 30 degrees and the ankle plantarflexed at 30 degrees with a cast for 6 weeks, and then gradually move and bear weight, avoiding strenuous exercise for 6 months.
