Diagnosis and treatment of dorsal scapular nerve entrapment syndrome

  The cause is due to anatomic compression of the dorsal scapular nerve or the cervical 5 nerve root in its pathway.  There are two possible causes of scapulodorsal nerve entrapment: one is compression of the cervical nerve root, especially the cervical 5 nerve root, which involves the scapulodorsal nerve, which is a branch of the nerve; the other is compression of the scapulodorsal nerve due to anatomical factors in its course, such as the tendonous initiating fibers of the middle oblique muscle, thus, the scapulodorsal nerve entrapment is mostly present in thoracic outlet syndrome, but it can also exist alone.  History and symptoms 1.Common symptoms: This disease is common in young and middle-aged women, all patients have neck, shoulder and back discomfort and pain as the main symptoms, neck discomfort is related to the weather, rainy days, winter aggravation, can also be aggravated after exertion, upper arm posterior extension, lifting with a pulling sensation in the neck, neck, shoulder and back pain often make patients unable to sleep, patients feel uncomfortable how to put the affected limb, but can not clearly point out the site of pain.  2, less frequent symptoms: a few cases may have shoulder weakness, occasional hand numbness, mainly for the forearm and hand radial half numbness.  Signs and examinations Some patients may have decreased sensation in the forearm, a few patients have decreased muscle strength in the upper limbs, especially shoulder abduction, and there are obvious pressure points at the midpoint of the posterior border of the sternocleidomastoid muscle and 3cm next to the spinous process of the 3rd and 4th thoracic vertebrae.  Treatment plan: 1. Conservative treatment: First of all, consider conservative treatment, mainly local closure. One is the posterior edge of the midpoint of the sternocleidomastoid muscle, and the other is 3cm next to the spinous process of 3 and 4 thoracic vertebrae. 2. Surgical treatment: For those who are ineffective in conservative treatment or have severe symptoms of thoracic outlet syndrome, surgical treatment can be considered.
