It is difficult to treat dizziness, check from head to toe, and suffer from the pain of injections and medication, but it is the otoliths that cause trouble. There are many friends who are usually healthy, but suddenly feel dizzy when they get up in the morning or turn over in the middle of the night, accompanied by visual rotation, nausea and vomiting, and the symptoms are relieved in about a minute, after which similar situations occur repeatedly, often when they lie down, sit up, turn over and turn their heads, and in severe cases, they cannot turn over, or even for two or three days. Unable to get out of bed. When they came to the hospital, some of them said they had insufficient blood supply to the brain and some said they had cervical spondylosis, and they spent a lot of money on IV medication. We overlooked one of the most frequent diseases in vertigo – otoliths. Many patients can be diagnosed as “otoliths” through detailed medical history and physical examination, which is called “Benign Paroxysmal Positional Vertigo” (BPPV). The effect is immediate. Each of us has a very small organ in the ear called the vagus, which contains some circular structures (called the semicircular canals), the vestibule and the cochlea, which contains fluid and very fine hair-like receptors that are used to monitor the rotation of the head. The cochlea is the hearing receptor. The area between the semicircular canal and the cochlea is called the vestibule, which contains the ellipsoidal and balloon sacs to which the “otoliths” (calcium carbonate crystals) are attached and which are used to monitor the movement of the head and its position in space. There are a number of reasons why these crystals may detach from the ellipsoidal sac and mistakenly enter one of the semicircular canals (especially when you are lying down), which causes that semicircular canal to receive the wrong information about the change in position and therefore feel dizzy. This is the cause of what we call “benign paroxysmal positional vertigo (BPPV)”, which is caused by the dislodgement of calcium carbonate particles (otoliths) from the otolith membrane of the ellipsoidal sac and their entry into the semicircular canal. The onset is characterized by paroxysmal vertigo with characteristic nystagmus caused by changes in head position. In the past, it was said that it was generally more prevalent in people aged 50-70 years, and the ratio of female to male incidence was 1.5:1. However, according to our observation of more than 300 patients with BPPV, the disease can occur at any age, especially because of the increased stress in modern people, there is a clear trend of the disease becoming younger, and the prevalence is also high in 20-40 years old. Due to the lack of sufficient awareness and attention to benign paroxysmal positional vertigo, there is a very high rate of misdiagnosis and underdiagnosis. Therefore, these patients often do not receive effective treatment, and their symptoms recur, causing a lot of inconvenience to their work and life. Most of the cases are primary, but they can also be secondary to trauma, vestibular neuritis, or inadequate blood supply to the basilar artery. The only effective treatment for benign paroxysmal positional vertigo is repositioning treatment, which means that the otoliths that are out of their original position can be repositioned by changing the body posture. At present, some hospitals have carried out manual repositioning, but it is inefficient and the success rate is not high, so it is not widespread nationwide. Recently, a brand new method of resetting, fully automatic resetting treatment, has been created to make up for the shortcomings of manual resetting. Patients can conveniently examine and treat vertigo by simply sitting on the instrument and having the physician operate the computer to rotate it. We have treated more than three hundred patients. After three resetting operations, patients’ vertigo symptoms can be basically eliminated, and the cure rate can be over 95%. We hope that more people will learn about this disease and its best treatment method, the fully automatic reset treatment, so that more patients can be relieved of their ailments.
