What to do for post-operative ACL rehabilitation

  1.Immediate postoperative period – 2nd week after surgery:
  (1) Ultrashort wave therapy to reduce swelling.
  (2) Use straight knee brace for 12 weeks.
  (3) 30% weight-bearing with crutches until the 6th postoperative week.
  (4) Passive knee extension exercises.
  (5) Passive knee flexion exercises (CPM): start at postoperative week 3, passive knee flexion up to 90°-110 at postoperative week 4. Start wall gliding exercises when the patient can obtain 80-90° of knee flexion.
  (7) Move the patella.
  (8) Start quadriceps isometric contraction exercises and Kokonoe muscle strength exercises on the 1st postoperative day.
  2. Postoperative rehabilitation in the 3rd-6th week:
  (1) Continue to maintain knee extension and muscle strength exercises.
  (2) Mobility exercises: 5 weeks after surgery, passive knee flexion up to 130°.
  (3) Closed chain exercises (e.g. shallow squat, leg press) and straddle training will be started at 5 weeks.
  3. Post-operative rehabilitation from week 7-12:
  (1) The passive knee flexion and extension angle is the same as the healthy side, and there is no obvious pain and swelling.
  (2) The affected limb can walk with full weight bearing.
  (3) Gradually return to full squatting exercises with protection.
  (4) Begin kneeling exercises and moderate intensity resistance fixed bicycle exercises at 11 weeks.
  4. Postoperative rehabilitation from the 13th to the 18th week:
  (1) Continue to strengthen the quadriceps and Kokonoe muscle strength exercises.
  (2) Jogging can be considered when the joint is stable.
  (2) Start swimming, but forbid breast swimming in the early stage.
