The theory of opening the five organs originates from the Internal Canon. The five viscera are hidden inside and are interconnected with the five orifices. To know the internal organs, we must look at the external organs. Ear diseases are treated from the kidney, and nose diseases are treated from the lung. The “Treatise on Typhoid Miscellaneous Diseases” has laid down a complete system of theory, method, prescription and medicine in Chinese medicine, in which the six meridians are widely used not only for the treatment of external diseases, but also for the treatment of internal injuries. The six meridians can be clinically assigned to a certain meridian according to the location of the disease, the passage of the meridian and the malfunction of the meridian. Tinnitus and deafness are clinically common, often recurring, and very difficult to treat, so the correct clinical identification and use of medication is particularly important. Professor Chen is good at using meridian prescriptions in his treatment and has achieved good clinical results with flexible treatment. In the treatment of tinnitus and deafness, Prof. Chen believes that the focus is on “Tong” and is good at using “Shi Calamus” to open the orifices, and through different combinations to achieve different purposes of tonicity and diarrhea. 1. The kidney opens the orifice in the ear, and the heart also opens the orifice in the ear, nourishing the yin without forgetting to pass the yang. Qing. Zhang Luyun: “The ear is the orifice of the kidney and the main body of the foot Shao Yin meridian. However, the heart is also located in the ears …… cover the kidneys to treat the inner yin, the heart to treat the outer yang. In the way of heaven and earth, the essence is always changing, so the pure essence of the qi goes up to the empty orifice, and the ear is subject to it and hears Cong” (“Zhang’s Medical General”). This shows that although the ears are dominated by the kidneys, they also need the warmth of the heart yang. Gui Zhi Gan Cao Long Bone Oyster Soup is from the Treatise on Typhoid Fever and was originally used to treat excessive sweating and irritability due to deficiency of Heart Yang. Although the treatment of tinnitus and deafness is not mentioned, if we combine it with the original article 75: “Before holding the pulse, the patient’s hand forked from the heart. If the teacher tries to make him cough but he does not cough, he must be deaf and deaf-blind. This is because the sweating is heavy and deficient, so this is the case”. In his clinical treatment of deafness, Chen seized the main feature of “heart panic and pressure, excessive sweating” and combined it with the manifestations of cold limbs and atrophy of the mind, and used Gui Zhi Gan Cao Long Bone Oyster Soup, which was effective every time. 2. The clan veins converge in the ears, and the ears are also moistened by the liver, so the opening of the orifices should first nourish the blood. The kidney produces marrow, which leads to the brain. “If the marrow is insufficient, the brain will turn to tinnitus. Liver blood is also responsible for moistening the ear. If liver blood is deficient and does not provide enough nourishment, the ear veins will lose nourishment and the ear orifices will become deaf. As stated in the Selected Works of Ulcerology, “The liver meridian of Jueyin, blood deficiency and wind heat, or dry fire and wind heat in the liver meridian, can cause ear sores, which must cause internal heat and itching, tinnitus and deafness. Among them, angelica, Chuanxiong and peony nourish blood and soften the liver, blood is full then tinnitus and deafness from stopping; Chaihu dredges the liver, so that liver qi is unobstructed, liver blood is nourished, qi and blood are harmonized; blood deficiency produces heat with gardenia to clear drainage. Tianma “black eyes and head dizziness, wind deficiency within the work, non-Tianma can not be removed” (“Compendium of Materia Medica”); hooked vine “hand and foot syncope medicine. The foot Shaoyang meridian starts at the eye canthus, goes up to the corner of the head, goes down behind the ear, and enters the ear ……. In such cases, Chen often adds Chai Hu and Scutellaria as adjuvants or directly uses Xiao Chai Hu Tang plus flavor. Case 1: Fu, female, 48 years old, a housewife, first diagnosed on January 3, 1994. The kidney is the main organ of the ear, and the heart is also open to the ear. The disease is caused by the displacement of the heart and the loss of the heart’s yang, so the formula is to tonify the heart and warm it, and to calm the mind. Eight months later, the patient’s tinnitus recurred due to excessive fatigue from playing mahjong, and his hearing was diminished. He had to consult the doctor again, seeing that his moss was clear and his mouth was not dry and bitter. The treatment was still in accordance with the original method of tonifying the heart and promoting Yang, with the addition of Phellodendron 10g. Case 2: Yao, male, 57 years old, farmer, first seen on August 20, 1993. He had been ill for half a month with a cold and fever, sore throat, followed by ringing in the right ear like a cicada, with few intervals, accompanied by right migraine, swelling and pain in the right gum and cheek, not daring to eat and chew. His body temperature rose in the afternoon, reaching a maximum of 39.1℃. Could not sleep at night due to tinnitus and toothache. After using antibacterial, anti-inflammatory and pain-relieving western medicine and stacking of antipyretic and detoxifying agents, except for a slight reduction of swelling and pain in the throat, the other symptoms not only did not decrease but also worsened. The patient is suffering from redness and swelling of the right cheek and the root of the tooth, painful to touch, difficulty in opening the mouth, thin yellowish greasy coating on the tongue with grayish hood, and a stringent pulse with number. Radix Bupleurum 8g, Radix Scutellariae 8g, Radix Fructus Panax 8g, Radix Asclepias 12g, Rhizoma Huanglian 3g, Radix Sinensis 3g, Rhizoma Chuanxiong 20g, Radix Angelicae Dahuricae 10g, Acorus Calamus 10g, Radix Glycyrrhiza Uralensis 6g, 3 doses. The pain was relieved on the day of medication, and he was able to sleep at night, and the tinnitus sound was slightly lighter. The patient has not relapsed after 1 year of follow-up. The patient’s deficiency of heart yang and loss of heart yang, poor sleep and dreaminess, and red tongue without moss are obviously signs of yin injury, so lily, Wu Wei Zi and Bai Zi Ren were added to nourish yin. Bai Wei is used to treat the deficiency of irritability, and Hepatitis Bark to calm the mind and Gan Song to awaken the stomach. The patient’s tinnitus and deafness became more and more severe when taking Shu Di, Radix Angelicae Sinensis, Fructus Lycii and Wu Wei Zi. The reason for this is that he knew that the ear is the main organ of the kidney, but did not know that it is also the heart organ. As soon as he saw the Yin injury, he abused the nourishing and thick medicines, and failed to take into account the Yang side of the heart. As a result, not only is it difficult to restore the yin fluid, but the remaining yang energy is even more injured and the orifice is especially closed. Therefore, Chen advocates nourishing without being greasy, nourishing yin without forgetting to promote yang, and combining ventilation with tonicity. In Case 2, the patient’s tinnitus and headache were on one side, the ear was swollen in front and behind, and the pulse was stringent, which was due to the congestion of Shao-Yang evil qi. In this formula, Chai Hu and Scutellaria baicalensis are the main herbs of Xiao Chai Hu Tang, which can clear the heat of Shao Yang and act as a guide to the meridians. They are also used to clear heat and detoxify the body, and are complemented by Hsiang Xin, Chuanxiong, Bai Zhi and Calamus to relieve pain. The medicine and make half a month of the disease, a dose of knowledge, seven doses of healing. The former doctor cleared heat and detoxification but not effective, is because the location of the disease is not identified, not to support the cause. The effect of the treatment became apparent after following Zhongjing’s method. The human body is an organic whole, and the theory of five organs and five orifices is not a mechanical relationship. In his clinical practice, Chen emphasized specific analysis of specific problems. The tinnitus and deafness described in this article was cured by treating the heart and liver and gallbladder.
