During the winter and summer holidays, there are always many parents inquiring about the treatment options for their child’s prepuce or circumcision, i.e., the surgical options. In circumcision, the foreskin covers the entire head of the penis and the opening of the foreskin is narrow and cannot be turned up. In circumcision, the foreskin completely covers the head of the penis, there is no local narrowing, and the foreskin can be turned up to reveal the head of the penis smoothly without restriction. Both of them can be treated surgically if they are accompanied by repeated local infections or discomfort in urination. Usually, the best age for surgery for domestic children with prepuce is around school age (6-8 years old), and the age of surgery for circumcision is mostly chosen after puberty. Commonly used surgical procedures include circumcision and circumcision. The former is to remove the distal foreskin stenosis ring and excess foreskin, suture the inner and outer plates, while the latter is to place a ligature ring inside the foreskin and then ligate and fix the foreskin with a thread loop on the surface, relying on blocking blood flow to make the distal foreskin ischemic and necrotic. There are advantages and disadvantages to both types of surgery. Second, the impact of local blood flow is different: 1, circumcision, the severed end of the vein cut, the distal excess foreskin was removed, reflux is not obstructed, no bruising at the severed end; severed end of the artery cut, the proximal blood supply is obstructed, the severed end is congested, tissue edema itself is not serious but serious hematoma. 2.Circumferential ligation, severed vein at the severed end, distal reflux obstruction, bruising at the severed end; tissue edema itself is more severe than circumferential ligation, but there is no hematoma. The nature of inflammatory reaction is different: 1, circumcision mainly causes plasma hemorrhagic reaction, which occurs immediately after surgery but lasts for a short time. 2.Circumcision mainly causes ischemic necrotic reaction, which occurs slowly but lasts for a long time after surgery. The length of the operation is different: 1. The operation time of circumcision is about 20-30 minutes. 2.Circumferential ligation takes about 5 minutes. In addition, the age and local development of the child should be taken into account. If the child is young, the penis is small, the foreskin has no obvious relative excess and the local stenosis is serious, circumcision is recommended, and vice versa, circumcision is chosen.
