Chinese medicine treatment for mouth ulcers

       Mouth ulcers are common diseases, often related to viral infections, vitamin deficiency or autoimmunity, etc. Most of them can heal on their own, but they are prone to recurrence.   I often use the method of inducing fire to return to the original source to treat recurrent oral ulcers. This kind of patients often have a special fear of cold in winter, cold extremities and difficult to warm, and they feel that they are particularly prone to fire and are prone to mouth ulcers, sore throat and other discomfort when they eat some fried food.   I. Overview of mouth ulcers
  (A) the characteristics of mouth ulcers
  In terms of characteristics, there are three aspects.
  1, the conscious symptoms: the general situation will be painful, there is no pain, this does not hurt the mouth ulcer, but to cause us enough attention. It may be a manifestation of some immune diseases.
  2, he perceived symptoms: refers to the doctor and other people can see, generally found in the mouth and nasal cavity inside some ulcers.
  3, recurrent: another feature of mouth ulcers is the recurrent attacks. Some people have recurrent mouth ulcers once a month, some people have them twice a month, and some people have them once every six months or a year.
  (B) Etiology of mouth ulcers
  The etiology is explained in two aspects: Chinese medicine and Western medicine. Chinese medicine is explained in terms of deficiency and actual. Western medicine starts from infectious and non-infectious. The infectious ones include bacteria and viruses. The non-infectious ones include self-inflicted causes and vitamin deficiency. Of course, sometimes the causes of mouth ulcers are not clear to us, and sometimes we have no way to start.
  The actual and deficiency in Chinese medicine include many. The deficiency symptoms should be distinguished from the meaning of Yin deficiency, Yang deficiency and Qi deficiency. The causes of real symptoms may be fire toxicity, stomach fire, qi stagnation, heart fire, damp heat, etc. As a TCM practitioner, it is important to first distinguish between deficiency and actuality, but also to be aware that the disease may be a combination of deficiency and actuality. This will be broken down in detail later in the treatment of symptoms.
  Second, the dialectical treatment of oral ulcers
  (A) Identification of oral ulcers
  From the perspective of Chinese medicine, the treatment needs to be dialectical, and the treatment includes internal and external treatment.
  For dialectical evidence, it is necessary to firstly identify the deficiency and reality, and secondly to identify the internal organs, whether they belong to the spleen, stomach, liver or heart, or are intermingled with each other. This mind map also implies some content, such as deficiency inside the Yin deficiency fire, or due to Yang deficiency and Qi deficiency caused by the Yin fire disease, manifested as cold in winter, cold hands and feet, but there are particularly easy to get on fire, a little bit of spicy food will be swollen and sore throat, which is common in the clinical and particularly prone to mis-treatment.
  Guangdong people especially like to drink herbal tea, XXX advertising, fire on the drink xxx, which misleads many people. First of all, we should remind everyone that we must look at our own constitution, whether it is deficient or real, deficient do not just drink herbal tea.
  To determine the physique, it is very important to be sure to know whether you are afraid of cold in winter, whether your hands and feet are cold, if so, there are also some manifestations of fire, such as mouth ulcers, face acne, sore throat, etc., then you tend to be a case of yin fire. If your face to grow acne, throat to be red and swollen, mouth ulcers, this kind of is deficiency fire, then do not just use the method of clearing heat and detoxification, that is only effective at the time, the long-term result is to hurt their own Yang Qi, causing deficiency fire more obvious.
  In terms of real evidence. There may be cases of fire toxicity, damp heat, liver stagnation and qi stagnation. For fire toxicity, the manifestation is a red tongue with red dots, a slippery pulse, yellow tongue coating, and mouth ulcers that are often painful and burning; for damp-heat, the tongue is red, the tongue coating is yellow and greasy, and the pulse is slippery. For people with liver depression and qi stagnation, temper tends to be violent, moody and prone to temper tantrums.
  After talking about the deficiency and reality of the disease, in the clinical judgment, we also need to know that he is the internal organs of the problem. For example.
  Spleen and stomach: people with heavy stomach fire, bad breath, thirst, bitter mouth. Poor appetite, rotten stools, and a pale tongue when the spleen is deficient in dampness. Spleen and stomach damp heat may have bad breath, the tongue is red, the moss is yellow and greasy, and the pulse is slippery.
  Liver and Kidney: Liver and Kidney deficiency or Yin deficiency is often accompanied by fever in the five hearts, fever in the palms of the hands, fever in the feet, and weakness of the waist and legs. In kidney-yang deficiency, the tongue is pale and fat, with tooth marks on the edge of the tongue. In the case of kidney yin deficiency, the legs and feet are weak.
  Heart: The heart is mainly an overactive heart fire, which manifests itself as a mouth ulcer that grows on the tip of the tongue and is more painful.
  In fact, in the clinic, the real and virtual, the internal organs are often intermingled with each other, and sometimes there are cases of liver depression, spleen deficiency, dampness, or liver depression and yin fire.
  (B) Treatment of mouth ulcers.
  The internal treatment of Chinese medicine is based on your diagnosis to develop a rule of treatment, and thus the choice of prescription.
  For example, Chai Hu Gui Zhi Gan Jiang Tang is suitable for patients with liver-depression and spleen-deficiency. Patients may be more irritable, have bad temper, get upset and vomit easily, which is a manifestation of liver-depression; spleen-deficiency is manifested by poor appetite, pale tongue and loose stools, which can then be considered using Chai Hu Gui Zhi Gan Jiang Tang.
  The second formula is Fire Inducing Soup, which is a formula from Chen Shiduo’s “Dialectical Record”, with five herbs: Shu Di, Mai Dong, Fu Ling, Ba Ji Tian, and Wu Wei Zi. This is very good for the treatment of mouth ulcers. This formula is suitable for yin-fire evidence, especially yin-fire caused by yang deficiency. In the winter, the hands and feet are cold, and the mouth ulcers are often long. Then this method we use is called drawing fire back to the source. There is a misconception here, most patients and even doctors think that a long mouth ulcer is a fire and clearing the fire will be enough, but it is not true. This is a kind of deficiency fire, which is due to Yang Qi floating on top and on the surface, resulting in more localized and upper fire in the mouth, and Yang Qi is not latent, resulting in less fluid and leading to mouth ulcers. The treatment at this point is not simply to put out the fire, but to pull this fire down to replenish our kidney yang, so that the body’s yin and yang are balanced. I think the method of drawing the fire back to the source is a positive treatment for mouth ulcers, and using this method does not make the patient’s body worse and worse. In the clinic, many patients, acne, mouth ulcers, herpes simplex, etc., use Huang Lian Shang Qing Wan and other heat-clearing and detoxifying drugs, these drugs may be effective within a short period of time, but long-term use is likely to make the patient’s Yang deficiency symptoms more and more serious, the condition is prone to recurrence. For girls, a more serious condition of Yang deficiency may be dysmenorrhea, which is caused by blood stasis from drinking herbal tea and getting cold, and pain if it does not pass.
  I have attached a formula to Gui Huo Tang – Pei Zi Li Zhong Wan, which can also treat yin-fire syndrome by strengthening the spleen and warming the yang, and is preferred when the symptoms of yang deficiency are particularly severe. Even more tonic formulas such as Si Wei Tang are sometimes chosen to warm Yang. These are all based on the patient’s specific situation to choose the most suitable formula.
  The most important thing is that it is very familiar with Zhi Bai Di Huang Tang, which is nourishing Yin and clearing heat, suitable for Yin deficiency and fire. These patients are thin, not afraid of cold, and a little afraid of heat, with red tongue coating, dry mouth, and thin pulse.
  The stomach heat hurts the fluids, that is, our fire is relatively large, and over time it tends to hurt our fluids, at this time we use bamboo leaf and gypsum soup to add or subtract.
  Of course, in clinical practice, these few remedies are not enough, there are many other remedies that can be used, such as Huang Lian Agaricus Tang, etc. I won’t go over them together here.
  For a patient, what are the external medications besides the internal ones? Throat wind dispersion, watermelon cream, and tintype dispersion spray can be used.
  For patients with recurring mouth ulcers, in clinical practice I usually use self-blood therapy, extracting two milliliters of venous blood from the patient and injecting one milliliter on each side of the foot three miles, one to two times a week, eight times in a row for a course of treatment, in most cases the patient will slowly improve.   What exactly does autologous blood therapy do? It is suitable for many skin diseases, especially allergic skin diseases, and some weak constitution. Foot San Li is an important health point, and there is a saying that “one needle is equivalent to eating three old hens”. Why does self-blooding have an effect? There will be a local stimulation response after self-blooding, which is equivalent to acupuncture every day. The mechanism is that our blood contains some things that are sensitive to the outside world and will produce antibodies to the sensitive things. When we draw the blood out and pump it back inside the human body, the body will think it is something foreign to recognize and tolerate, producing a secondary immune response and achieving an anti-allergic effect.
  The third external treatment method is acupuncture, which is still very effective in treating some mouth ulcers. Especially for patients who are irritable and depressed, this time acupuncture often has an immediate effect.
  Three, mouth ulcers daily care
  (a) mental care.
  I think many diseases nowadays have emotional and mental problems in them. It is important to find out from the patient with mouth ulcers whether he or she is under stress or irritable. We need to channel the patient’s emotions so that we can have improved outcomes. For all of us, if you usually have irritability, it means you have liver qi stagnation, qi is not smooth, this is the time to see Chinese medicine has immediate effect.
  Of course, in addition to using herbal medicine to de-stress the liver, we also have to teach patients some ways to de-stress their emotions. Sometimes we will recommend patients to read some books to learn by themselves and regulate their emotions. We say that the main solution to the problem of emotions still depends on you personally, others can only help you, and your own regulation is very important.
  (B) Dietary contraindications.
  Reasonable diet first of all to identify the body. Can not be the clouds, must pay attention to distinguish between the real and the virtual, this is very important, if you do not know how to find a Chinese doctor to distinguish you. For example, Yang deficiency patients are afraid of cold hands and feet in winter, so people should not eat so much fruit. Fruits are mostly cold, such as kiwi, grapes, apples, etc., cold body people are not recommended to eat. You can eat some hot fruits such as dragon fruit and cinnamon. For patients with dampness and heat, such as fire and red tongue with yellow coating, it is helpful to eat some fruits that can replenish vitamins and relieve mouth ulcers at this time. For some patients with entrained dampness they should eat foods that dispel dampness, such as barley and mung beans.
  (iii) Good sleep.
  Sleep is also very important for the treatment of mouth ulcers. Chinese medicine emphasizes sleeping on the meridian, Chinese medicine believes that the yang energy of people is constantly alternating in the process, there is an alternation at noon time and an alternation at night. 11:00 p.m. to 1:00 p.m. is the process of Yang going into Yin. If we, at this time, have not gone to bed and are still working, then our yang energy is running and cannot be hidden. At this time, Yang Qi will float on the top, float on the surface, Yang Qi floating on the top will form fire, fire can cause us to get mouth ulcers. Another thing is not only to go to bed early but also to pay attention to the quality of sleep, whether it is easy to fall asleep, sleep deeply or dream throughout the night, whether it is easy to wake up during sleep, whether it is easy to fall asleep after waking up, etc.

  Some patients are constipated, and toxins in the body will definitely accumulate in this case, usually caused by heat toxicity, and deficiency may also lead to inability to pass stools.   (E) adhere to the treatment.
  In particular, some skin diseases should be adhered to for three months, six months and some even two or three years.
